温肾培元降毒汤结合中药灌肠治疗慢性肾功能衰竭临床观察

Clinical Observation on Wenshen Peiyuan Jiangdu Decoction Combined with Chinese Medicine Enema in the Treatment of Chronic Renal Failure

目的 探讨温肾培元降毒汤结合中药灌肠对慢性肾功能衰竭患者肾功能及血清IL-6、IL-8、CRP水平的影响。方法 以北票市中医院收治的40例慢性肾功能衰竭患者为研究对象,选取时间为2019年3月—2020年1月,按随机数字表法分为对照组和观察组,各20例。对照组采用常规治疗联合前列地尔,观察组在对照组基础上采用温肾培元降毒汤结合中药灌肠治疗。1周为一个疗程,2组均治疗4个疗程。比较2组治疗后临床疗效、治疗前后肾功能及炎性因子水平。结果 治疗后,观察组总有效率为75.00%,高于对照组的30.00%;将患者血尿素氮、血肌酐、血清CRP、IL-6、IL-8含量同治疗前比较发现,2组均呈降低趋势,其中与对照组相比,观察组指标下降幅度更大(P <0.05);将患者内生肌酐清除率同治疗前比较发现,2组均呈升高趋势,其中对照组与观察组相比,其呈降低趋势(P <0.05)。结论 在慢性肾功能衰竭中采取温肾培元降毒汤、中药灌肠联合治疗可明显降低血尿素氮、血肌酐水平,提高内生肌酐清除率,改善肾功能,同时可抑制机体炎症反应,疗效显著。

Objective To investigate the effects of Wenshen Peiyuan Jiangdu decoction combined with Chinese medicine enema on renal function and serum IL-6,IL-8 and CRP levels in patients with chronic renal failure.Methods 40 patients with chronic renal failure admitted to Beipiao Central Hospital were selected as the research objects.The selection time was from March 2019 to January 2020.According to the random number table,they were divided into a control group and an observation group,with 20 cases in each group.The control group was treated with conventional treatment combined with alprostadil,and the observation group was treated with Wenshen Peiyuan Jiangdu decoction combined with Chinese medicine enema on the basis of the control group.One week was a course of treatment,and both groups have 4 courses of treatment.The clinical efficacy,renal function and inflammatory factor levels before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the observation group was 75.00%,which was higher than 30.00% of the control group.The blood urea nitrogen,blood creatinine,serum CRP,IL-6,and IL-8 levels of the patients were compared with those before treatment.Compared with the observation group,the control group showed an increasing trend.Comparing the patient’s endogenous creatinine clearance rate with that before treatment,it was found that both groups showed an increasing trend.Compared with the observation group,there was a decreasing trend in control group( P < 0.05).Conclusion Combination therapy with Wenshen Peiyuan Jiangdu decoction and Chinese medicine enema in chronic renal failure can significantly reduce blood urea nitrogen and blood creatinine levels,increase endogenous creatinine clearance,improve renal function,and inhibit the body’s inflammatory response,and have significantly therapeutic effect.
