终末期肾病肾性贫血患者甲状旁腺激素水平及临床意义

Level of Parathyroid Hormone in Patients with Renal Anemia Due to End-stage Renal Disease and Its Clinical Significance

目的分析甲状旁腺激素(PTH)在终末期肾病(ESRD)肾性贫血患者中的水平及临床意义。方法收集2019年6月—2021年1月收治的ESRD肾性贫血102例作为观察组,将同期收治的慢性肾病贫血患者100例作为对照组。检测2组血清全段甲状旁腺激素(iPTH)、血红蛋白(Hb)、血清钙、血肌酐(Scr)和血尿素(BUN)的含量,分析iPTH与ESRD肾性贫血患者肾功能及贫血严重程度的关系。结果观察组血清iPTH、钙、Scr、BUN较对照组明显升高,且Hb较对照组明显降低(P<0.01)。重度贫血组血清iPTH、钙、Scr、BUN较轻、中度贫血组明显升高,且Hb较轻、中度组明显降低(P<0.01)。血清iPTH与Scr、BUN呈正相关(r=0.591、0.643,P<0.01),与Hb呈负相关(r=-0.482,P<0.01)。iPTH对ESRD肾性贫血程度有较高的诊断效能。结论ESRD肾性贫血患者iPTH含量明显增高,血钙增加,Hb含量降低,加重ESRD肾性贫血程度,且iPTH对ESRD肾性贫血严重程度的诊断有指导意义。

Objective To analyze level of parathyroid hormone(PTH)in patients with renal anemia due to end-stage renal disease(ESRD)and its clinical significance.Methods A total of 102 patients with renal anemia due to ESRD admitted between June 2019 and January 2021 were collected as observation group,and other 100 patients with chronic renal anemia treated at the same period were selected as control group.Contents of serum intact parathyroid hormone(iPTH),hemoglobin(HB),serum calcium,serum creatinine(Scr)and blood urea(BUN)were detected,and correlations between iPTH with renal function and anemia severity in patients with renal anemia due to ESRD were analyzed in two groups.Results Compared with those in control group,serum contents of iPTH,calcium,Scr and BUN were significantly higher,while Hb content was significantly lower in observation group(P<0.01).In severe anemia subgroup,serum contents of iPTH,calcium,Scr and BUN were significantly higher,while Hb was significantly lower than those in mild and moderate anemia subgroups(P<0.01).Serum iPTH was positively correlated with Scr and BUN(r=0.591,0.643,P<0.01),and negatively correlated with Hb(r=-0.482,P<0.01).The iPTH had good efficacy in diagnosis of patients with renal anemia due to ESRD.Conclusion The content of iPTH is significantly increased,and content of blood calcium is increased,while content of HB is decreased in patients with anemia due to ESRD,which may aggravate the degree of renal anemia due to ESRD.In addition,iPTH has guiding significance for diagnosing the severity of renal anemia due to ESRD.
