不同分子分型乳腺癌雄激素受体表达差异的研究 被引量:1

摘要 目的:探讨不同分子分型乳腺癌雄激素受体(AR)的表达差异。方法:收集2019年3月至2020年9月间惠州市第三人民医院80例乳腺癌患者的石蜡包埋组织,使用免疫组织化学法及链霉菌康生物素蛋白–过氧化物酶连结法(SP)对AR在不同分子分型中表达进行观察,分析AR在不同乳腺癌分子分型中表达特征,分析乳腺癌组织中AR表达与各分子分型乳腺癌预后关系。结果:80例患者分子分型:管腔上皮A型(Luminal A)42例,管腔上皮B型(Luminal B)11例,人表皮生长因子受体2(HER2)过表达型10例,基底细胞样型(Basal–like)9例,正常细胞样型(Normal–like)8例;共有58例呈现AR阳性,其中又以Luminal A(80.95%)、Luminal B(72.73%)、Normal–like(75.00%)分型的阳性比例最高,Basal–like(44.44%)阳性比例最低。随访截止于2021年9月或死亡时间,Luminal A、Luminal B、Base–like与Normal–like的AR阳性组术后预后不良结局发生率均较阴性组更低,差异具有统计学意义(P<0.05)。HER2过表达型乳腺癌AR阳性组术后预后不良结局发生率与阴性组对比差异无统计学意义(P>0.05)。结论:AR在不同分子分型乳腺癌组织中表达存在显著差异,可以为临床分子分型判断及预后不良结局预测提供可靠依据。 Objective To investigate the difference of androgen receptor(AR)expression in different molecular types of breast cancer.Methods Paraffin embedded tissues from 80 patients with breast cancer in the Third People's Hospital of Huizhou from March 2019 to September 2020 were collected.Immunohistochemical staining and Streptomyces bioassay protein peroxidase linked method were used to observe the expression of AR in different molecular typps.The expression characteristics of AR in different molecular typing of breast cancer were analyzed,and the relationship between AR expression in breast cancer tissues and prognosis of breast cancer was analyzed.Results The molecular typing of 80 patients were 42 cases of Luminal epithelium type A(Luminal A),11 cases of Luminal epithelium type(Luminal B),10 cases of human epidermal growth factor receptor(HER2)overexpression,9 cases of basal cell-like type(Basal-like)and 8 cases of normal cell-like type(Normal-like).A total of 58 cases showed AR positive,with the highest proportion in Luminal A(80.95%),Luminal B(72.73%)and normal-like(75.00%),and the lowest in basal-like(44.44%).Follow-up up to September 2021 or the time of death,the incidence of postoperative adverse outcomes in Luminal A,Luminal B,Base-like and Normal-like AR positive groups were lower than those in the AR negative group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative adverse outcomes between the AR positive group and the AR negative group of HER2 overexpressed breast cancer(P>0.05).Conclusion There is significant difference in the expression of AR in different molecular types of breast cancer tissues,which can provide a reliable basis for the judgment of clinical molecular types and the prediction of poor prognosis.
