摘要
肺炎喘嗽作为小儿常见病,发病较急,临床表现以发热、咳嗽、气喘,以及咯痰痰鸣为主要特征,病机关键责之肺气郁闭,主要治法为宣肺开闭,化痰平喘,但临床诊疗中单独使用此法疗效差强人意。近年来诸多医家提出,“瘀证”贯穿小儿肺炎喘嗽发病全过程,并认为在其基础治则中应适当佐以化瘀,可加速病情恢复。本研究通过临床实践及查阅相关医案,发现采用清热化瘀法治疗热邪所致小儿肺炎喘嗽,临床疗效较好。鉴于此,本研究基于肺炎喘嗽不同时期热邪致瘀特点,结合临床上采用清热化瘀法治疗不同时期小儿肺炎喘嗽典型案例,按照小儿生理病理特点,探讨小儿肺炎喘嗽治疗各期佐以清热化瘀法的必要性及临床意义,以期为临床诊疗小儿肺炎喘嗽提供新的思路及方法。
As a common disease in children,the onset of pneumonia cough is acute.The clinical manifestations are mainly characterized by fever,cough,asthma and expectoration.Its key pathogenesis is lung qi stagnation,and the main treatment method is dispersing lung qi,removing phlegm and relieve asthma.However,the curative effect of this method alone is not satisfactory.In recent years,many doctors proposed that"stasis syndrome"occures during the whole process of pediatric pneumonia cough,and believe that dispersing stasis should be appropriately used in the treatment to accelerate the recovery of the disease.Through clinical practice and consulting relevant medical records,the author found that,the method of clearing heat and removing stasis has satisfactory curative effect on pediatric pneumonia cough caused by heat evil.Therefore,based on the characteristics of stasis caused by heat evil in different stages of pneumonia cough,combined with the physiological and pathological characteristics of children and the typical cases of pediatric pneumonia cough at different stages treated by the method of clearing heat and removing stasis,this article discusses the necessity and clinical significance of clearing heat and removing stasis in each stage of pediatric pneumonia cough,in order to provide new ideas and methods for clinical diagnosis and treatment.
