比较经直肠与经会阴前列腺穿刺活检术对前列腺癌的诊断价值

Comparing the Diagnostic Value of Transrectal and Transperineal Prostate Biopsy for Prostate Cancer

目的探究经直肠与经会阴前列腺穿刺活检术对前列腺癌的诊断价值。方法回顾性分析我院收治的330例前列腺癌待排需要行前列腺穿刺患者的临床资料,按照检查方式的不同分为会阴组(n=160)及直肠组(n=170)。以病理学结果为依据,比较经会阴与直肠超声引导前列腺穿刺活检诊断前列腺癌的临床价值;评估2组治疗期间,不良反应发生情况。结果160例患者经会阴超声引导前列腺穿刺活检诊断前列腺癌的灵敏度:77.78%(70/90)、特异度:71.43%(50/70)、诊断符合率75.00%120/160,阳性预测值77.78%(70/90),阴性预测值71.43%(50/70);170例患者经直肠超声引导前列腺穿刺活检诊断前列腺癌的灵敏度88.89%(80/90)、特异度62.50%(50/80)、诊断符合率76.47%(130/170),阳性预测值72.73%(80/110),阴性预测值83.33%(50/60)。结论经直肠与经会阴前列腺穿刺活检术在前列腺癌中的诊断率无差异,2者不良反应发生率无差异,操作者可以依据实际临床情况及患者个人情况选择合适的穿刺方式。

Objective To explore the diagnostic value of transrectal and transperineal prostate biopsy for prostate cancer.Methods A retrospective analysis of the clinical data of 330 patients with prostate cancer in our hospital who needed to undergo prostate puncture was divided into perineal group(n=160)and rectal group(n=170)according to different examination methods.Based on the pathological results,compare the clinical value of perineal and rectal ultrasound-guided prostate biopsy in the diagnosis of prostate cancer;Evaluate the occurrence of adverse reactions during the treatment of the two groups.Results The sensitivity of 160 patients with perineal ultrasound-guided prostate biopsy to diagnose prostate cancer:77.78%(70/90),specificity:71.43%(50/70),diagnostic coincidence rate 75.00% 120/160,positive predictive value 77.78%(70/90),the negative predictive value was 71.43%(50/70);the sensitivity of transrectal ultrasound-guided prostate biopsy for diagnosis of prostate cancer in 170 patients was 88.89%(80/90),specificity:62.50%(50/80),diagnosis coincidence rate:76.47%(130/170),positive predictive value 72.73%(80/110),negative predictive value 83.33%(50/60).Conclusion There is no difference in the diagnosis rate of transrectal and transperineal prostate biopsy in prostate cancer,and there is no difference in the incidence of adverse reactions between the two.The operator can choose the appropriate puncture method according to the actual clinical situation and the patient’s personal situation.
