摘要
目的对不同海拔地域驻训官兵跟骨超声骨量进行定量分析研究。方法通过收集资料完整的男性健康官兵200例,其中高海拔地域官兵95例,驻地海拔高度为2500~4000 m,低海拔地域官兵105例,驻地海拔高度低于1000 m。年龄19~42岁,无明确心脑血管疾病、未服用影响骨代谢药物。使用SONOST 3000定量超声式骨量检测仪对官兵进行右足跟骨超声骨量的测定,以T值作为评价指标。采用SAS 9.2以T值判断结果为应变量,根据超声骨量T值分为骨量正常组(T>-1)和骨量减少组(T≤-1),以年龄、官兵海拔类别为自变量,进行多因素Logistic回归分析,分析自变量对应变量的影响是否有统计学意义。结果Logistic回归分析结果说明不同海拔高度和年龄对T值影响有统计学意义。高海拔地域官兵T值低于低海拔地域官兵;高海拔地域官兵骨量减少发生概率高于低海拔地域官兵;官兵骨量减少与年龄呈正相关。结论将跟骨超声骨量作为窗口,预测评估全身骨量,对官兵定期开展超声骨量测定,树立训练伤超前预防理念,注重潜在风险管理,准确评估骨量特点,尽量减少伤情发生。
Objective To analyze quantitatively calcaneal ultrasonic bone mass of officers and soldiers stationed at different altitudes.Methods The complete data of a total of 200 healthy male officers and soldiers were collected,including 95 cases in high altitude area with an altitude of 2500~4000 m and 105 cases in low altitude area with an altitude of less than 1000 m.Aged 19~42 years,without definite cardiovascular and cerebrovascular diseases,and without taking drugs affecting bone metabolism.SONOST 3000 quantitative ultrasonic bone measurement instrument was used to measure the bone mass of right foot calcaneus with T value as evaluation index.SAS 9.2 was used as the dependent variable,and according to the ultrasonic bone mass T value,they were divided into normal bone mass group(T>-1)and reduced bone mass group(T≤-1).Age and altitude of officers and soldiers were used as independent variables,and multivariate Logistic regression analysis was conducted to analyze whether the influence of the corresponding variables of independent variables was statistically significant.Results Logistic regression analysis showed that different altitude and age had statistically significant influence on T value.T value of officers and soldiers in high altitude area was lower than that in low altitude area.The incidence of bone loss in high altitude area was higher than that in low altitude area.The decrease of bone mass was positively correlated with age.Conclusion Calcaneal ultrasound bone mass is used as a window to predict and evaluate the whole body bone mass.Ultrasound bone mass measurement is carried out regularly for officers and soldiers,the concept of advanced prevention of training injuries is established,the potential risk management is emphasized,and bone mass characteristics are accurately evaluated to minimize the occurrence of injuries.
