三种手术方式治疗青光眼合并白内障疗效比较

Comparison of the efficacy of three surgical methods for the treatment of glaucoma combined with cataracts

目的比较3种手术方式用于青光眼合并白内障的价值。方法对2019—2021年间本科室接诊单眼青光眼合并白内障病患90例随机均分3个组。甲组采取白内障超声乳化吸除术联合人工晶体植入术,乙组采取白内障超声乳化吸除术、人工晶体植入术联合虹膜周边切除术,丙组采取白内障超声乳化吸除术、人工晶体植入术联合小梁切除术。对比3个组视力、眼压、并发症发生率。结果术后视力恢复率丙组97%,比甲组77%、乙组80%高(P<0.05),但甲组和乙组相近(P>0.05)。术后眼压丙组(13.26±1.47)mmHg,比甲组(14.92±2.01)mmHg、乙组(14.73±1.96)mmHg低(P<0.05),但甲组和乙组相近(P>0.05)。并发症发生率丙组3%、乙组10%,都比甲组33%低(P<0.05),但丙组和乙组10%相近(P>0.05)。结论青光眼合并白内障中联用白内障超声乳化吸除术、人工晶体植入术与小梁切除术有利于并发症预防、眼压降低及视力改善。

Objective To study the value of three surgical methods in glaucoma complicated with cataract.Methods FromJuly 2019 to June 2021,90 patients with glaucoma and cataract were admitted,all of whom were monocular.They were randomly divided into 3 groups.Group a received phacoemulsification combined with intraocular lens implantation,group B received phacoemulsification combined with intraocular lens implantation combined with peripheral iridectomy,and group C received phacoemulsification combined with intraocular lens implantation combined with trabeculectomy.Visual acuity,intraocular pressure and complication rate were compared.Results The recovery rate of postoperative visual acuity in group C was 97%,which was higher than 77% in group A and 80% in group B(P<0.05),but there was no significant difference between in group A and group B(P>0.05).For postoperative intraocular pressure,group C(13.26±1.47)mmHg was lower than group A(14.92±2.01)mmHg and group B(14.73±1.96)mmHg(P<0.05),but there was no significant difference between group A and group B(P>0.05).In terms of the incidence of complications,3% in group C and 10% in group B were lower than 33%in group A(P<0.05),but there was no significant difference between in group C and group B(P>0.05).Conclusion The combination of phacoemulsification,intraocular lens implantation and trabeculectomy in glaucoma complicated with cataract is conducive to the prevention of complications,the reduction of intraocular pressure and the improvement of visual acuity.
