化痰消瘀方治疗慢性萎缩性胃炎伴肠上皮化生的疗效分析

Analysis of the efficacy of resolving phlegm and antistasis in the treatment of chronic atrophic gastritis with intestinal metaplasia

目的探究化痰消瘀方治疗慢性萎缩性胃炎(CAG)伴肠上皮化生的临床疗效。方法2015—2017年在本院经影像学及病理检查确诊为CAG并伴有肠上皮化生患者200例,随机分为实验组和对照组,对照组80例,观察组120例。对照组予以维酶素片+吗丁啉片+法莫替丁治疗,观察组予以化痰消瘀方治疗,均治疗12周。采用ELISA法检测血清炎症因子白介素6(IL-6)和肿瘤坏死因子α(TNF-α),比较治疗效果和不良反应情况。结果与治疗前相比,患者经过治疗中医症状积分、组织病理评分、血清IL-6及TNF-α水平均降低(P<0.05);且观察组中医症状积分、组织病理评分、血清IL-6及TNF-α水平均低于对照组(P<0.05)。两组治疗过程中均未出现明显的不良反应。结论化痰消瘀方治疗CAG伴肠上皮化生可降低体内炎症水平,改善胃黏膜肠上皮化生。

Objective To explore the clinical effect of huatan xiaoyu prescription on chronic atrophic gastritis with intestinal metaplasia.Methods A total of 200 patients with chronic atrophic gastritis complicated with intestinal metaplasia confirmed by imaging and pathological examination from February 2015 to December 2017 were collected and randomly divided nto control group and observation group by simple number method,80 cases and 120 cases,respectively.The control group was treated with vertase tablets+morpholine tablets+famotidine.The observation group was treated with huatan Xiaoyu prescription for 12 weeks.Serum levels of inflammatory cytokines interleukin-6(IL-6)and tumor necrosis factorα(TNF-α)were detected by ELISA,and treatment effects and adverse reactions were compared between the two groups.Results Compared with before treatment,TCM symptom score,histopathological score,serum IL-6 and TNF-αlevels in 2 groups were significantly decreased after treatment(P<0.05);After treatment,TCM symptom score,histopathological score,serum IL-6 and TNF-αlevels in the observation group were significantly lower than those in the control group(P<0.05);There were no obvious adverse reactions in both groups.Conclusion Huatan Xiaoyu prescription has significant clinical efficacy in the treatment of chronic atrophic gastritis with intestinal metaplasia,which can significantly reduce the level of inflammation in vivo and improve the intestinal metaplasia of gastric mucosa,with high safety.
