静脉血栓栓塞症综合介入治疗对休克指标的影响

The effect of Venous thromboembolism combined interventional treatment of shock

目的分析高危肺栓塞伴下肢深静脉血栓(DVT)患者行综合介入治疗后对动脉血氧分压、休克指数指标的影响。方法选取2018—2021年间我院收治34例高危型肺栓塞伴DVT患者,均行综合介入治疗干预,对比治疗前后患者休克指数、动脉血氧分压等指标改变。结果术后1 d的休克指数和血氧饱和度分别为(0.72±0.20)、(20.16±3.54)次/min,低于术前的(0.96±0.10)、(28.25±3.01)次/min,差异有统计学意义(P<0.05)。术后1 d血氧分压和血氧饱和度分别为(80.61±1.24)mmH、(97.45±1.25)%,高于术前(69.72±7.35)mmH、(87.02±2.11)%,差异有统计学意义(P<0.05)。术后1d患者休克指数、呼吸频率低于术前,而血氧分压、血氧饱和度水平高于术前(P<0.05)。结论对于高危肺栓塞伴DVT患者采取综合介入治疗,可有效提高机体动脉血氧分压水平,降低休克指数。

Objective To analyze the effects of comprehensive interventional therapy on arterial oxygen partial pressure and shock index in patients with high-risk pulmonary embolism with lower extremity deep venous thrombosis(DVT).Methods 34 patients with high-risk pulmonary embolism with DVT treated in our hospital from 2018 to 2021 were treated with comprehensive access treatment intervention.The changes of shock index and arterial oxygen partial pressure before and after treatment were compared.Results The shock index and blood oxygen saturation were(0.72±0.20)and(20.16±3.54)times/min,respectively,one day after operation,which were significantly lower than those before operation(0.96±0.10),(28.25±3.01)times/min(87.02±2.11)%,(P<0.05),the difference was statistically significant(P<0.05).The results showed that the shock index and respiratory rate were lower than those before operation,while the levels of PO2 and SpO2 were significantly higher than those before operation(P<0.05).Conclusion Comprehensive interventional therapy for patients with high-risk pulmonary embolism and DVT can effectively improve the level of arterial oxygen partial pressure and reduce the shock index.
