摘要
目的:判断无牙颌患者不戴[牙合]堤发音时面下1/3最小距离与语音法确定适宜的垂直距离后戴[牙合]堤发音时的面下1/3最小距离是否一致,并进一步研究二者的关系。方法:选取需进行双颌或单颌总义齿修复的无牙颌患者54例,用语音法确定垂直距离。在患者鼻下及颏下各标记一点,分别在戴[牙合]堤前后与患者交谈,自然带入S音,用直尺测量患者鼻下与颏下标记点间最小距离,分别记录为Before、After值,并将After和Before值做差,得出差异值。将Before和After值进行比较,并根据患者垂直距离丧失时间、无牙颌时间、性别、年龄、单双颌无牙颌等分组,对各组差异值进行比较。结果:Before值与After值间差异有统计学意义(P<0.001),后者比前者平均值高,差异值为0-3 mm的比例为87.04%,大于3 mm的比例为11.11%,差异值为负的比例为1.85%。垂直距离丧失大于一年与小于等于一年两组间差异值有差异,具有统计学意义(P<0.05),而无牙颌时间、性别、年龄、单双颌无牙颌等各组间差异值的差异没有统计学意义(P>0.05),且各因素间均没有交互作用(P>0.05)。结论:语音法确定合适的垂直距离后戴[牙合]堤比不戴[牙合]堤发音时面下1/3最小距离高。
Objective:The aim of this study was to compare the minimal distances of the 1/3 lower face with and without occlusion rims using phonetic tests of the vertical dimension,and to study the correlation between them.Methods:A total of 54 patients with the single or double edentulous jaws were recruited,the vertical dimension was determined using the phonetic tests.After marking the subnasale and gnathion,the dentist talked with the patients with and without occlusion rims and observed when the/s/sound is enunciated spontaneously,measured the minimal distance between the subnasale and gnathion on the 1/3 lower face,recorded as the Before and After values respectively.After subtracting the Before values were the difference values.Compared the Before values with the After values,and compared the difference values according to the time of loss of vertical dimension,the time of edentulous jaws,gender,age,and single or double edentulous jaws.Results:There was significant difference between the Before and After values(P<0.001),the mean values of After were higher than those of Before.The percentage of patients whose difference values were 0-3 mm,greater than 3 mm and negative were 87.04%,11.11%and 1.85%respectively.There was significant difference between the difference values of two groups of patients whose time of loss of vertical dimension were greater and less than 1 year(P<0.05).There was no significant difference between the difference values of groups of patients according to the time of edentulous jaws,gender,age,and single or double edentulous jaws(P>0.05).Conclusion:The minimal distances of the 1/3 lower faces with occlusion rims using phonetic tests of the vertical dimension were higher than those distances without occlusion rims.
