新方法制作上颌骨中空式赝复体的临床应用

The clinical application of a new method for making hollow maxillary prosthesis

目的:探讨一种更快捷的上颌骨缺损中空式赝复体的修复方法,为患者提供更简便有效的治疗。方法:选择济宁医学院附属湖西医院2018年1月至2021年4月就诊的上颌骨缺损修复的患者,按照纳入排除标准选择40例一侧上颌骨缺损的患者为研究对象,按数字表法随机分两组,实验组20例用新方法制作上颌骨赝复体修复缺损部分;对照组20例用传统方法制作赝复体修复缺损部分。在患者戴用赝复体调[牙合]前,分别对每位患者戴赝复体进行咬合关系检查,记录正中、侧方、前伸早接触点数据;在患者戴用赝复体2周后,分别对每位患者戴赝复体进行咀嚼效率、语音清晰度值、吞咽功能检测,记录数据,进行统计数据分析。结果:实验组患者的早接触点个数、咀嚼效率、语音清晰度、吞咽功能正常例数(5.24±4.75,0.624±0.168,0.630±0.054,19、95%),与对照组(9.53±5.54,0.518±0.161,0.595±0.052,13、65%)相比,差异有统计学意义(t=2.629,2.037,2.088,X^(2)=3.90,均P<0.05)。结论:新方法制作中空式赝复体修复上颌骨缺损更简便有效。

Objective:To explore a faster repair method of maxillary defect hollow prosthesis,and to provide patients with more convenient and effective treatment.Methods:The patients who were treated for maxillary defect repair at the Huxi Hospital of Jining Medical College were selected from January 2018 to April 2021.According to the inclusion and exclusion criteria,40 patients with one maxillary defect were selected as the research objects,who were randomly divided into two groups according to the number table method.In the experimental group,20 cases were subjected to the maxillary prosthesis using the new method;In the control group,20 cases were subjected to the maxillary prosthesis using the traditional method.Before wearing of the prosthesis,every prosthesis was checked for the occlusal relationship,and the median,lateral,and early contact points were recorded;2 weeks after wearing of the prosthesis,the chewing efficiency,speech intelligibility value,and swallowing function were tested and recorded,and the statistical data was also analyzed.Results:The number of early contact points,chewing efficiency,speech intelligibility,and normal swallowing function of patients in the experimental group(5.24±4.75,0.624±0.168,0.630±0.054,19,95%) were compared with those of the control group(9.53±5.54,0.518±0.161,0.595±0.052,13,65%),the difference was statistically significant(t=2.629,2.037,2.088,X^(2)=3.90,all P<0.05).Conclusion:The new method of making hollow prostheses to restore maxillary defects is more convenient and effective.
