摘要
目的探讨超声弹性成像在距腓前韧带损伤中的应用价值。方法选取2018年1~12月就诊的外踝距腓前韧带损伤的72例患者为研究对象。所有患者接受常规超声以及超声弹性成像检查,将手术病理结果作为判定金标准,比较患者弹性成像的杨氏模量值与剪切波速度,分析两种检测的成像质量以及检测的敏感性及特异性。结果完全撕裂组患者的杨氏模量值与SWV均低于部分撕裂、挫伤以及正常组(P<0.05);超声弹性成像的Ⅰ级成像质量优于对照组,而Ⅲ级成像质量低于对照组(P<0.05);超声弹性成像检测的敏感性为90.0%(54/60)、特异性83.3%(10/12),均高于常规超声检测的70.0%(42/60)、50.0%(6/12),差异有统计学意义(χ^(2)=5.037,4.986,P<0.05)。结论对距腓前韧带损伤患者采用超声弹性成像检测,有助于提升诊断的符合率,成像质量、敏感性及特异性均相对较高,促使诊断的准确性显著提高,利于临床应用及推广。
Objective To investigate the application value of ultrasound elastography in diagnosis of anterior talofibular ligament injury.Methods A total of 72 patients with external malleolus anterior talofibular ligament injury who were treated in our hospital from January 2018 to December 2018 were enrolled in the study.All the patients underwent conventional ultrasound and ultrasound elastography,and the surgical pathological results were taken as gold standard to compare the Young’s modulus value and shear wave velocity.The imaging quality as well as its sensitivity and specificity were observed and compared between the two detection methods.Results The Young’s modulus value and SWV in patients with complete tear injury were significantly lower than those in patients with partial tear injury,and those in contusion group and control group(P<0.05).Ultrasonic elastographyⅠlevel image quality was superior to that in control group,however,Ⅲlevel imaging quality was inferior to that in control group(P<0.05).The sensitivity and specificity of ultrasound elastography were 90.0%(54/60)and 83.3%(10/12),respectively,which were significantly higher than those(70.0%,42/60)and 50.0%(6/12)by conventional ultrasound(P<0.05).Conclusion The application of ultrasound elastography in the detection of anterior talofibular ligament injury is helpful to improve the diagnose accordance rate and imaging quality,and its sensitivity and specificity are relatively higher,so as to improve the diagnosis accuracy,thus,which is worthy of clinical application and promotion.
