摘要
目的:探讨用新型DR数字化分析方法进行骨质疏松研究的可行性。方法:1.回顾分析本院在一年之内接受两次DR检查的289例正常成人的影像资料,应用后处理软件对两次DR影像分别进行感兴趣区的像素值测量,同时选取特定区域作为校正区并测量像素值,两次像素值的比值称为像素校正值,观察两组像素校正值有无差异。2.选取本院25例对髋部同时进行了DXA和DR检查的患者的影像资料,观察DXA测量值与DR像素校正值之间的关系。3.使用DR像素校正值参与患者的临床决策。结果:1.两组像素校正值比较,差异无统计学意义(P>0.05),表明经过本方法校正,像素校正值固定不变。2.髋部DR像素校正值与DXA测量值呈正相关关系(股骨颈、大粗隆、Ward区r值分别为0.453、0.506、0.830,P值均<0.05),且诊断准确度较好(股骨颈、大粗隆、Ward区曲线下面积分别为0.831、0.792、0.851,P值均<0.05)。表明DR像素值校正值具备与DXA相近的诊断价值。3.在实际工作中,DR像素校正值的变化与患者的临床表现吻合,可作为临床决策的参考。结论:对DR影像经过数字化分析得出的像素校正值可用于骨质疏松的研究,具备一定的可行性,可作为临床决策的参考。
Objective:This paper introduces a new method of bone mineral density measurement with digital measurement technology and proves it helpful for clinical decision-making.Methods:1.The study subjects include the DR results of 289 normal adults who underwent two DR examinations within one year in our hospital.The pixel values of the region of interest were measured for the two examination results,and the fixed region was selected as the correction region and the pixel value was measured.The ratio between the two is called the pixel correction value,and the difference between the two sets of pixel correction values is observed.2.25 patients in our hospital who underwent both DXA and DR examinations of the hip were selected to observe the relationship between them.3.Try to use DR pixel correction value to participate in the clinical decision-making of some patients.Results:1.There was no significant difference between the two groups of DR pixel correction values(P>0.05),indicating that there was no difference between the two groups of pixel correction values after correction by this method.2.The hip DR correction value was positively correlated with the DXA bone density value(r=0.453,0.506,0.830,P<0.05),and the diagnostic accuracy was significant(AUC=0.831,0.792,0.851,P<0.05.It indicates that the DR pixel value correction value has a diagnostic value similar to that of DXA.3.In practice,the change in the DR pixel correction value is consistent with the patient’s clinical performance and can be used as a reference for clinical decision making.Conclusion:After the DR image is measured by the post-processing workstation,the DR pixel value correction value can be used to osteoporosis researches.It has positive clinical significance and can be used as a reference for clinical decision-making and has high promotion value.
