PETCT与HRCT鉴别诊断周围型肺癌的价值比较

Value Comparison of PETCT and HRCT in Differential Diagnosis of Peripheral Lung Cancer

目的:比较正电子发射计算机断层显像(PETCT)与高分辨率CT(HRCT)鉴别诊断周围型肺癌(PLC)的价值。方法:选择2016年1月-2020年12月于我院治疗的疑似为PLC患者132例。入选者均接受PETCT与HRCT检查。分析病理穿刺结果,以病理穿刺诊断结果作为金标准,计算PETCT与HRCT单项及联合诊断PLC特异度、敏感度及准确率。结果:132例疑似为PLC患者经病理穿刺证实PLC有110例,其中鳞癌、腺癌各有28例、82例;非PLC患者22例,其中肺纤维瘤、结核球及炎性肉肿各有3例、4例、15例;PETCT+HRCT诊断PLC敏感度及准确率高于PETCT、HRCT,差异有统计学意义(P<0.05);PETCT+HRCT与PETCT、HRCT诊断PLC特异度对比,PETCT与HRCT诊断PLC敏感度、特异度及准确率对比,差异无统计学意义(P>0.05)。结论:PETCT与HRCT联合使用鉴别诊断PLC敏感度及准确率高于PETCT与HRCT,有利于提高PLC检出率,降低漏诊率,为PLC患者早期诊治提供指导。

Objective:To compare the value of positron emission computed tomography(PETCT)and high resolution CT(HRCT)in the differential diagnosis of peripheral lung cancer(PLC).Methods:132 suspected PLC patients treated in our hospital from January 2016 to December 2020 were selected.All participants underwent PETCT and HRCT.The pathological puncture results were analyzed.Taking the pathological puncture diagnosis results as the gold standard,the specificity,sensitivity and accuracy of single and combined diagnosis of PLC by PETCT and HRCT were calculated.Results:there were 110 cases of PLC confirmed by pathological puncture,including 28 cases of squamous cell carcinoma and 82 cases of adenocarcinoma;There were 22 non PLC patients,including 3 cases of pulmonary fibroma,4cases of tuberculoma and 15 cases of inflammatory sarcoma;The sensitivity and accuracy of PETCT+HRCT in the diagnosis of PLC were higher than those of PETCT and HRCT(P<0.05);There was no significant difference in the sensitivity,specificity and accuracy of PETCT+HRCT in diagnosing PLC compared with PETCT and HRCT(P>0.05).Conclusion:the sensitivity and accuracy of the combined use of PETCT and HRCT in the differential diagnosis of PLC are higher than those of PETCT and HRCT,which is helpful to improve the detection rate of PLC,reduce the missed diagnosis rate,and provide guidance for the early diagnosis and treatment of PLC patients.
