摘要
目的:探讨常规MRI联合3DCE-MRV在脑静脉窦血栓形成中的应用价值。方法:回顾性分析26例脑静脉窦血栓形成患者的临床及影像学资料。结果:脑静脉窦血栓共26例,其中上矢状窦7例,横窦3例,乙状窦2例,直窦1例,2个及2个位置发生血栓者13例。T1WI呈高信号22例,低信号4例;T2WI及FLAIR呈高信号20例,低信号6例;DWI呈高信号20例,等信号6例;3D增强表现为静脉窦内低信号的充盈缺损;3DCE-MRV表现为静脉窦局部中断或缺失。脑实质水肿10例,表现为T1WI低信号、T2WI高信号;急性脑梗死8例,DWI表现为高信号;蛛网膜下腔出血2例,表现为脑沟内条片状T1WI高信号、T2WI低信号;7例患者在3D增强扫描时表现为脑膜及小脑幕强化。结论:常规MRI平扫联合3DCE-MRV可以提高CVST诊断的准确性及全面性,对临床早期诊断、指导治疗具有重要意义,值得临床推广应用。
Objective:To explore the application value of conventional MRI combined with 3DCE-MRV in cerebral venous sinus thrombosis.Methods:Retrospective analysis of the clinical and imaging data of 26 patients with cerebral venous sinus thrombosis.Results:There were 26 cases of cerebral venous sinus thrombosis,including 7 cases of superior sagittal sinus,3 cases of transverse sinus,2 cases of sigmoid sinus,1 case of straight sinus,and 13 cases of thrombosis in 2 and 2 locations.T1WI showed high signal in 22 cases and low signal in 4 cases;T2WI and FLAIR showed high signal in 20 cases and low signal in 6 cases;DWI showed high signal in 20 cases and isosignal in 6 cases;3D enhancement showed low signal filling defect in venous sinus;3DCE-MRV showed local interruption or loss of venous sinuses.Parenchymal edema in 10 cases showed low signal on T1WI and high signal on T2WI;8 cases of acute cerebral infarction showed high signal on DWI;2 cases of subarachnoid hemorrhage showed high signal on T1WI and low T2WI in the sulcus Signal;7 patients showed enhancement of meninges and tentorium during 3D enhanced scan.Conclusion:Conventional MRI combined with 3DCE-MRV can diagnose cerebral venous sinus thrombosis early and accurately,and it is worthy of clinical application.
