关节镜下小切口联合补阳还五汤对肩袖损伤患者的治疗效果研究

Study on the efficacy of arthroscopic assisted small incision combined with Buyang Huanwu Decoction on patients with rotator cuff injury

目的研究关节镜下小切口联合补阳还五汤对肩袖损伤患者的治疗效果。方法选取2020年1月至2021年1月在浙江中医药大学第二临床医学院就诊的肩袖损伤患者80例,随机分为对照组40例,联合组40例。对照组患者给予关节镜下小切口修补术和依托考昔治疗,联合组在对照组的基础上给予补阳还五汤治疗。比较两组的GM-CSF水平、肩关节外旋、外展、内旋活动度、VAS评分及治疗总有效率。结果联合组手术后GM-CSF水平低于对照组(P<0.05)。联合组患者肩关节外旋、外展、内旋活动度均高于对照组,差异有统计学意义(P<0.05)。联合组手术后AESE、UCLA、Constant-Murely评分高于对照组,差异有统计学意义(P<0.05)。术后两组患者VAS评分均低于术前(P<0.05);联合组手术后VAS评分低于对照组,差异有统计学意义(P<0.05)。结论关节镜下小切口联合补阳还五汤能够有效降低患者的GM-CSF水平,提高治疗有效率,促进患者恢复。

Objective To study the efficacy of arthroscopic assisted small incision combined with Buyang Huanwu decoction on patients with rotator cuff injury.Methods A total of 80 patients with rotator cuff injury who attended the Second Clinical Medical College of Zhejiang Chinese Medical University from January 2020 to January 2021 were selected and randomly divided into the control group(n=40)and the combined group(n=40).Patients in the control group were treated with etoricoxib and arthroscopic assisted small incision repairing technique,while the combined group was treated with Buyang Huanwu Decoction on the basis of the treatment in the control group.The levels of granulocyte-macrophage colony-stimulating factors(GM-CSF),external rotation,abduction and internal rotation mobility of shoulder joints,Visual Analogue Scale(VAS)score and total effective rate of treatment of both groups were counted.Results GM-CSF levels in the combined group were lower than those in the control group after surgery(P<0.05).The external rotation,abduction and internal rotation mobility of the shoulder joints were higher in the combined group than those in the control group,with statistically significant differences(P<0.05).The score of American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form(AESE),University of California at Los Angeles(UCLA)activity score and Constant-Murely score were higher in the combined group than those in the control group after surgery,with statistically significant differences(P<0.05).The postoperative VAS scores of the two groups were lower than those before surgery(P<0.05),and those of the combined group were lower than those of the control group,with statistically significant differences(P<0.05).Conclusion The combination of arthroscopic assisted small incision and Buyang Huanwu Decoction can effectively reduce the GM-CSF levels,improve the efficacy and promote the recovery of patients.
