ICU护士健康工作环境与患者安全文化氛围感知的相关性研究

Correlation between ICU nurses’healthy work environment and patients’perception of safety cultural atmosphere

目的了解重症监护室(ICU)护士健康工作环境与患者安全文化氛围感知现状,并探索两者的相关性。方法选取上海市第一人民医院和上海市红房子妇产科医院的222名ICU护士为调查对象,采用一般情况调查表、美国危重症护理学会(American Association of Critical Care Nurses,AACN)健康工作环境评估量表及ICU患者安全测评量表进行调查。结果ICU护士AACN健康工作环境评估量表得分为(71.23±13.18)分,ICU患者安全测评量表得分为(136.27±20.33)分。相关性分析显示:ICU患者安全中的安全氛围、工作满意度、安全资源、沟通交流、团队合作和交接班维度与ICU健康工作环境各维度呈正相关(P<0.01);ICU患者安全中的害怕受罚维度与ICU健康工作环境中价值的认可和专业的沟通维度呈正相关(P<0.05)。结论ICU的健康工作环境对ICU患者安全文化氛围感知具有重要影响,有必要建立良好的沟通氛围,加强对护士工作价值的认可,发挥管理者的领导力,促进ICU内形成良好的患者安全文化氛围,提升患者安全结局。

Objective To understand the current situation of between intensive care unit(ICU)nurses’healthy work environment and patients’perception of safety cultural atmosphere,and explore the correlation of them.Methods A total of 222 ICU nurses from 2 general hospitals in Shanghai were investigated by using general condition questionnaire,American Association of Critical Care Nurses(AACN)Healthy Work Environment Assessment Tool and the Assessment Scale for Patient Safety Culture in ICU.Results The score of AACN Healthy Work Environment Assessment Tool for ICU nurses was(71.23±13.18)points,and the score of the Assessment Scale for Patient Safety Culture for ICU patients was(136.27±20.33)points.Correlation analysis showed that the dimensions of safety atmosphere,job satisfaction,safety resources,communication,teamwork,and shift change in the safety culture of ICU patients were positively correlated with the dimensions of healthy work environment(P<0.05).The dimension of fear of being punished in the safety culture of ICU patients was positively correlated with the value recognition and professional communication in ICU healthy work environment(P<0.05).Conclusion ICU healthy work environment has an important impact on ICU patients’perception of safety cultural atmosphere.It is necessary to establish a good communication atmosphere,strengthen the recognition of nurses’work value,give play to the leadership of managers,promote the formation of a good patient safety culture atmosphere in ICU,and improve the safety outcome of patients.
