新医改后糖尿病患者住院费用分析

Analysis of Diabetes Hospitalization Cost after the New Medical Reform

目的分析新医改后糖尿病患者的住院费用情况,为降低糖尿病患者疾病负担提供循证依据。方法收集西安市某县2011—2018年出院诊断为糖尿病患者的新农合住院费用全部数据,采用描述性分析、单因素分析等方法探究糖尿病住院患者住院费用及其影响因素。结果共收集6588人次糖尿病住院患者费用数据。2011—2018年间,糖尿病住院患者人次和住院总费用显著增加,次均药占比则从2011年的50.1%下降至2018年的38.1%;年龄、性别、住院天数是住院自费费用的影响因素(P<0.05)。结论新医改后新农合糖尿病患者人次及住院费用快速增长,药品费用得到有效控制。降低住院天数、提高男性患者用药依从性以及加强40岁以下人群的血糖监测,对于减轻糖尿病患者经济负担具有重要意义。

Objective To provide evidence for decreasing patients'disease burden with diabetes by analyzing the expenditures of diabetic inpatients after the new healthcare reform.Methods The hospitalization expenses data of inpatients diagnosed as diabetics in a county of Xi'an,during 2011 to 2018,was collected from the new rural cooperative medical system(NCMS).Descriptive analysis and univariate analysis were used to analyze the hospitalization expenditures and explore possible influencing factors.Results A total of 6588 cases of diabetics were collected.During 2011 to 2018,the number of inpatients and the total hospitalization expenses increased significantly,while the percentage of drug expenditure decreased from 50.1%in 2011 to 38.1%in 2018.Age,gender and hospital stay had a significant impact on the out-of-pocket expense(P<0.05).Conclusion The total hospitalization expenses and the number of inpatients are going through fast growth after the new medical reform,while the pharmaceutical cost has been effectively controlled.Reducing hospital stay,improving medication compliance of male patients and strengthening glucose monitoring for people under 40 years are essential strategies to decrease the disease burden for diabetics.
