体外膈肌起搏联合电针治疗慢性阻塞性肺疾病的临床研究

Clinical Study of External Diaphragm Pacing Combined with Electroacupuncture in the Treatment of Chronic Obstructive Pulmonary Disease

目的:探讨膈肌起搏联合电针治疗对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者肺功能及生活质量的影响。方法:将72例稳定期COPD患者随机分成对照组和观察组,每组36例,2组患者均给予常规内科治疗,观察组同时予体外膈肌起搏(external diaphragm pacer,EDP)治疗及电针,治疗前及治疗4周后检测各组患者肺功能,同时采用使用经典呼吸问卷CAT(2009年SGRQ的创始人琼斯教授研发)评估患者的生活质量。结果:观察组经4周治疗后,用力肺活量(FVC)、第一秒用力快速呼气量(FEV1)和FEV1与预期值比(FEV1%),1秒率(FEV1/FVC),较治疗前均明显改善,且均优于对照组,差异均有统计学意义(P<0.05),观察组经4周治疗后,CAT总分等较治疗前明显降低,且均好于对照组,差异均有统计学意义(P<0.05)。结论:膈肌起搏联合电针治疗能提高慢性阻塞性肺疾病患者肺通气功能,改善其生活质量。

Objective:To investigate the effect of diaphragm pacing combined with electroacupuncture on lung function and quality of life in patients with chronic obstructive pulmonary disease(COPD).Methods:72 patients with stable COPD were randomly divided into control group and observation group,36 cases in each group.Both groups were given routine medical treatment,while the observation group was given external diaphragm pacer(EDP)therapy and electroacupuncture at the same time.,before treatment and after 4 weeks of treatment,the lung function of patients in each group was detected,and the quality of life of patients was assessed by using the classic respiratory questionnaire CAT(developed by Professor Jones,the founder of SGRQ in 2009).Results:After 4 weeks of treatment in the observation group,the forced vital capacity(FVC),forced rapid expiratory volume in one second(FEV1),the ratio of FEV1 to expected value(FEV1%),and the 1-second rate(FEV1/FVC)were significantly higher than those before treatment.After 4 weeks of treatment,the total score of CAT in the observation group was significantly lower than that before treatment,and both were better than those in the control group,the difference was all Statistically significant(P<0.05).Conclusion:Diaphragmatic pacing combined with electroacupuncture can improve pulmonary ventilation function and quality of life in patients with chronic obstructive pulmonary disease.
