滋水涵木方对高龄卵巢储备功能下降患者子宫血流参数、内分泌功能及体外受精周期结局的影响

Effects of Zishui Hanmu Recipe on Uterine Blood Flow Parameters,Endocrine Function and Outcome of in Vitro Fertilization Cycle in Elderly Patients with Decreased Ovarian Reserve Function

目的:观察滋水涵木方对高龄卵巢储备功能下降患者子宫血流参数、内分泌功能及体外受精周期结局的影响。方法:将76例高龄卵巢储备功能下降患者按照随机数字表法分为治疗组和对照组,每组各38例。对照组给予体外受精-胚胎移植技术治疗,治疗组在对照组治疗基础上给予滋水涵木方。观察子宫血流参数和内分泌功能指标的改善情况,并比较两组临床疗效、体外受精周期结局、妊娠率及活产率。结果:①治疗组有效率为87.5%,对照组有效率为62.5%,两组临床疗效比较,差异有统计学意义(P<0.05)。②治疗后,两组患者收缩期峰值流速(peak systolic flow rate,PSV)和舒张末期血流速度(end diastolic velocity,EDV)水平均高于治疗前(P<0.05),阻力指数(resistance index,RI)低于治疗前(P<0.05);治疗组PSV和EDV水平均高于同期对照组(P<0.05),RI值低于同期对照组(P<0.05)。③治疗后,两组患者卵泡刺激素(follicle-stimulating hormone,FSH)和FSH/黄体生成素(luteinizing hormone,LH)水平均低于治疗前(P<0.05),雌二醇(estradiol,E_(2))水平高于治疗前(P<0.05);治疗组FSH和FSH/LH水平均低于同期对照组(P<0.05),E_(2)水平高于同期对照组(P<0.05)。④治疗组治疗后获卵数、成熟卵数、受精数、优质胚胎数、妊娠率及活产率均明显高于对照组(P<0.05)。结论:滋水涵木方能够调节高龄卵巢储备功能下降患者子宫血流参数,改善内分泌功能和体外受精周期结局,疗效确切。

Objective:To observe the effects of Zishui Hanmu formula on uterine blood flow parameters,endocrine function and the outcome of in vitro fertilization cycle in elderly patients with decreased ovarian reserve function.Methods:76 elderly patients with decreased ovarian reserve function were randomly divided into treatment group and control group,with 38 cases in each group.The control group was treated with in vitro fertilization embryo transfer technology,and the treatment group was treated with Zishui Hanmu formula on the basis of the treatment of the control group.The improvement of uterine blood flow parameters and endocrine function indexes were observed,and the clinical efficacy,in vitro fertilization cycle outcome,pregnancy rate and live birth rate were compared between the two groups.Results:①The effective rate was 87.5%in the treatment group and 62.5%in the control group;there was significant difference between the two groups(P<0.05).②After treatment,the levels of peak systolic flow rate(PSV)and end diastolic velocity(EDV)in the two groups were higher than those before treatment(P<0.05),and the resistance index(RI)was lower than that before treatment(P<0.05);the levels of PSV and EDV in the treatment group were higher than those in the control group(P<0.05),and the RI value was lower than that in the control group(P<0.05).③After treatment,the levels of follicle-stimulating hormone(FSH)and FSH/luteinizing hormone(LH)in the two groups were lower than those before treatment(P<0.05),and the levels of estradiol(E_(2))were higher than those before treatment(P<0.05);the levels of FSH and FSH/LH in the treatment group were lower than those in the control group(P<0.05),and the level of E_(2) was higher than those in the control group(P<0.05).④The number of eggs,mature eggs,fertilization,high-quality embryos,pregnancy rate and live birth rate in the treatment group were significantly higher than those in the control group(P<0.05).Conclusion:Zishui Hanmu formula can regulate the uterine blood flow parameters of elderly patients with decreased ovarian reserve function,improve endocrine function and the outcome of in vitro fertilization cycle.
