针灸联合康复运动应用于脑卒中后肩手综合征的效果

Effect of Acupuncture Combined with Rehabilitation Exercise on Shoulder and Hand Syndrome after Stroke

目的探讨针灸联合康复运动应用于脑卒中后肩手综合征(SHSAS)的效果。方法选取项城市中医院2019年1月至2021年2月就诊的SHSAS患者115例,根据就诊序号采用电脑随机数字表法分为A组(n=38)、B组(n=38)、C组(n=39)。三组均接受常规治疗,A组加用针灸,B组加用康复运动方案,C组加用针灸联合康复运动方案,比较三组疗效、上肢运动功能、VAS评分、水肿程度、日常生活能力。结果C组有效率97.44%高于A组(76.32%)、B组(71.05%),差异有统计学意义(P<0.05);治疗后C组上肢被动活动范围、上肢体运动功能、MBI、腕手关节运动功能评分均高于A组、B组,且B组上肢被动活动范围、上肢体运动功能、MBI、腕手关节运动功能评分高于A组,差异有统计学意义(P<0.05);治疗后C组VAS评分低于A组、B组,且A组VAS评分低于B组,差异有统计学意义(P<0.05);治疗后C组水肿程度均低于A组、B组,且B组水肿程度低于A组,差异有统计学意义(P<0.05)。结论针灸联合康复运动治疗SHSAS患者效果显著,可有效减轻疼痛、水肿症状,提升上肢运动功能,改善日常生活能力。

Objective To investigate the effect of acupuncture combined with rehabilitation exercises applied to post-stroke shoulder-hand syndrome(SHSAS)and the effect on VAS scores.Methods A total of 115 patients with SHSAS visited our hospital from January 2019 to February 2021 were selected and divided into groups A(n=38),B(n=38),and C(n=39)according to the visit serial number using the computerized random number table method.all three groups received conventional treatment,group A added acupuncture,group B added rehabilitation exercise program,and group C added acupuncture combined with rehabilitation exercise program.The efficacy,upper limb motor function,VAS score,edema degree and daily living ability of 3 groups were compared.Results The total effective rate of group C was 97.44%higher than that of group A(76.32%)and group B(71.05%)(P<0.05);the passive range of motion of upper limb,upper limb body motor function,MBI,and wrist-hand joint motor function scores of group C were higher than those of group A and group B after treatment,and the passive range of motion of upper limb,upper limb body motor function,MBI,and wrist-hand joint motor function scores of group B were higher than those of group A,the difference was significant(P<0.05);the VAS scores in group C were lower than those in groups A and B after treatment,and the VAS scores in group A were lower than those in group B,the difference was significant(P<0.05);the degree of edema in group C was lower than those in groups A and B after treatment,and the degree of edema in group B was lower than that in group A(P<0.05).Conclusion Acupuncture combined with rehabilitation exercise is effective in treating SHSAS patients,which can effectively reduce pain and edema symptoms,enhance upper limb motor function and improve daily living ability.
