系统化疼痛护理管理在GartlandⅢ型肱骨髁上骨折患儿中的应用效果

Effect of systematic Pain Nursing Management applied to Children with Gartland TypeⅢhumeral supracondylar Fracture

目的探讨系统化疼痛护理管理在GartlandⅢ型肱骨髁上骨折患儿中的应用效果。方法选择2019年3月至2021年2月郑州市骨科医院收治的210例GartlandⅢ型肱骨髁上骨折患儿为研究对象,利用随机数字表法将其分为两组,观察组106例,对照组104例,对照组采用常规护理,观察组实施系统化疼痛护理管理,比较两组患儿的疼痛程度[儿童疼痛行为量表(FLACC评分)]、肘关节功能康复时间、肘关节活动度、并发症发生情况及患儿训练依从度。结果术前两组患儿FLACC评分差异无统计学意义(P>0.05);术后12 h、24 h、48 h,两组FLACC评分均降低,且观察组FLACC评分低于对照组(P<0.05);护理1个月后,观察组肘关节功能康复时间短于对照组,观察组肘关节活动度大于对照组(P<0.05);护理期间,观察组并发症发生率为13.21%,低于对照组的25.96%(P<0.05);观察组训练依从度为96.23%,低于对照组的85.58%。结论系统化疼痛护理管理够有效缓解患儿疼痛程度,明显缩短肘关节功能康复时间,扩大肘关节活动度,降低并发症发生率,同时提高患儿训练依从度,值得临床借鉴。

Objective To explore the application effect of systematic pain nursing management in children with Gartland typeⅢsupracondylar fracture of the humerus.Methods From March 2019 to February 2021,210 children with Gartland III supracondylar fractures of the humerus were selected as the research objects,the random number table was used to divide them into two groups,106 cases in the observation group were giving systematic pain nursing management,104 cases in the control group were giving routine care.The pain degree[Child Pain Behavior Scale(FLACC scores)],elbow joint function rehabilitation time,elbow joint range of motion,occurrence of complications,and the children’s training compliance were compared between the two groups.Results There was no significant difference in FLACC scores between the two groups of children before operation(P>0.05).At 12h,24h and 48h postoperatively,the FLACC scores of the two groups decreased,and the FLACC score of the observation group was lower than that of the control group(P<0.05).After 1 month of nursing,the recovery time of elbow function in the observation group was shorter than that in the control group,and the range of motion of elbow in the observation group was greater than that in the control group(P<0.05).During the nursing period,the incidence of complications in the observation group was 13.21%,which was lower than 25.96%in the control group(P<0.05).The training compliance of the observation group was 96.23%,which was lower than 85.58%of the control group(P<0.05).Conclusion Systematic pain nursing management can effectively relieve the pain of children,significantly shorten the recovery time of elbow joint function,expand the range of elbow joint motion,reduce the incidence of complications,and improve the compliance of children with training.It is worthy of clinical reference.
