支架置入联合腹腔镜手术治疗左半结肠癌合并肠梗阻患者的效果

Effects of stent placement combined with laparoscopic surgery on patients with left colon cancer complicated with intestinal obstruction

目的:观察支架置入联合腹腔镜手术治疗左半结肠癌合并肠梗阻患者的效果。方法:回顾性分析2018年2月至2020年2月该院收治的80例左半结肠癌合并肠梗阻患者的临床资料,按照治疗方法不同分为对照组(n=35)和研究组(n=45)。对照组行腹腔镜手术治疗,研究组在对照组基础上联合支架置入治疗,比较两组手术相关指标水平、治疗前后血清肿瘤标志物水平及并发症发生率。结果:研究组术中出血量少于对照组,手术时间、肛门排气时间、住院时间均短于对照组,差异有统计学意义(P<0.05);术后,研究组血清甲胎蛋白和癌胚抗原水平均低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为4.44%,低于对照组的22.86%,差异有统计学意义(P<0.05)。结论:支架置入联合腹腔镜手术治疗左半结肠癌合并肠梗阻患者,可减少术中出血量,改善术中和术后恢复指标水平,降低血清肿瘤标志物水平和并发症发生率,其效果优于治疗单纯腹腔镜手术治疗。

Objective:To observe effects of stent placement combined with laparoscopic surgery on patients with left colon cancer complicated with intestinal obstruction.Methods:The clinical data of 80 patients with left colon cancer complicated with intestinal obstruction who were admitted to our hospital from February 2018 to February 2020 were retrospectively analyzed.According to different treatment methods,they were divided into control group(n=35)and study group(n=45).The control group was treated with laparoscopic surgery,while the study group was treated with stent placement on the basis of that of the control group.The intraoperative and postoperative recovery index levels,the serum tumor marker index levels before and after the surgery,and the incidence of complications were compared between the two groups.Results:The intraoperative blood loss in the study group was less than that in the control group;the operation time,the anal exhaust time,and the hospitalization time were shorter than those in the control group;and the differences were statistically significant(P<0.05).After the surgery,the serum alphafetoprotein and carcinoembryonic antigen levels in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the study group was 4.44%,which was lower than 22.86% in the control group,and the difference was statistically significant(P<0.05).Conclusions:Stent placement combined with laparoscopic surgery in the treatment of the patients with left colon cancer complicated with intestinal obstruction can reduce the amount of intraoperative blood loss,improve the levels of intraoperative and postoperative recovery indexes,and reduce the levels of serum tumor markers and the incidence of complications.Moreover,it is superior to single laparoscopic surgery.
