超声引导下腹横肌平面阻滞在妇科腹腔镜手术中的应用

Application of Ultrasound Abdominal Transverse Muscle Plane Block in Gynecological Laparoscopic Surgery

目的分析超声引导下腹横肌平面阻滞在妇科腹腔镜手术中的应用效果。方法选取我院2021年4月至2022年4月在我院就诊的100例择期妇科腹腔镜手术患者,按数字随机表法分为两组,即对照组50例,采用全身麻醉;观察组50例,采用超声引导下腹横肌平面阻滞复合全身麻醉;比较两组患者术后镇痛效果、手术麻醉情况。结果观察组切皮时、气腹建立、拔管时HR、MAP高于对照组(P<0.05)。观察组术后2、4、8、12 h的疼痛程度低于对照组,术后瑞芬太尼、丙泊酚使用剂量及24 h曲马多补救次数低于对照组(P<0.05)。观察组术后苏醒时间、拔管时间、肛门排气时间短于对照组,术后与麻醉相关的不良反应发生率(8.00%)低于对照组(26.00%)(P<0.05)。结论超声引导下腹横肌平面阻滞在妇科腹腔镜手术中具有较高的应用意义,术后镇痛效果好。

Objective To analyze the application effect of AST in gynecological laparoscopic surgery.Methods The 100 patients with elective laparoscopic gynecological surgery in our hospital from April 2021 to April 2022 were divided into two groups:50 control groups with general anesthesia,50 observation groups and ultrasound-guided abdominal transverse muscle floor block composite general anesthesia;postoperative analgesia and surgical anesthesia between the two groups.Results HR and MAP in the observation group were higher than those in the control group at the time of skin incision,establishment of pneumoperitoneum and extubation(P<0.05).The pain degree of the observation group at 2,4,8 and 12 hours after operation was lower than that of the control group,and the doses of remifentanil and propofol and the times of tramadol rescue at 24 hours after operation were lower than those of the control group(P<0.05).The postoperative recovery time,extubation time and anal exhaust time of the observation group were shorter than those of the control group,and the incidence of postoperative adverse reactions related to anesthesia(8.00%)was lower than that of the control group(26.00%)(P<0.05).Conclusion Ultrasound transverse plane blockade has high application significance in gynecological laparoscopic surgery and has good postoperative analgesic effect.
