血液透析串联血液灌流治疗维持性血液透析患者中综合护理模式的效果研究

Study on the Effect of comprehensive Nursing Model in Hemodialysis Patients with Maintenance Hemodialysis Treated by Tandem Hemoperfusion

目的观察血液透析(HD)串联血液灌流(HP)治疗维持性血液透析病患阶段实施综合护理模式的成效。方法选择2017年10月至2019年8月期间于我院接受HD串联HP治疗的90例维持性血液透析病患,随机分为Ⅰ、Ⅱ组,每组均为45例,分别给予常规护理、综合护理模式,分别检测各组患者治疗前后尿素氮(BUN)、甲状旁腺素(PTH)、白蛋白(Alb)、血红蛋白(Hb)水平、治疗后营养状态及治疗依从性,并在组间对比分析。结果治疗后,Ⅱ组患者BUN、PTH低于Ⅰ组,Alb、Hb水平高于Ⅰ组,差异有统计学意义(P<0.05);Ⅱ组营养状态改善总有效率、治疗依从性分别为95.56%、95.56%高于Ⅰ组71.11%、77.78%,差异均有统计学意义(P<0.05)。Ⅱ组的护理满意度为93.33%高于Ⅰ组的73.33%,差异有统计学意义(P<0.05)。结论维持性血液透析患者在接受HD串联HP治疗阶段,推行综合护理模式,和常规护理相比较,能更好的改善营养状态,提升患者治疗依从性,以及患者及家属护理满意度,值得推广。

Objective To observe the effect of comprehensive nursing mode in the treatment of patients with maintenance hemodialysis(HD)by tandem hemoperfusion(HP).Methods Ninety cases of maintenance hemodialysis patients who received HD tandem HP treatment in our hospital from October 2017 to August 2019 were randomly divided into group Ⅰ and group Ⅱ,with 45 cases in each group.They were given conventional nursing(group Ⅰ)and comprehensive nursing mode(group Ⅱ)respectively.The levels of urea nitrogen(BUN),parathyroid hormone(PTH),albumin(Alb),hemoglobin(Hb),nutritional status and treatment compliance were detected before and after treatment in each group,and comparative analysis was conducted between groups.Results After treatment,BUN and PTH in group Ⅱ were lower than those in group Ⅰ,Alb and Hb were higher than those in group Ⅰ,the differences were statistically significant(P<0.05).The total effective rate and treatment compliance of nutritional status improvement in group Ⅱ were 95.56% and 95.56%,respectively,higher than that in group Ⅰ(71.11% and 77.78%),with statistical significance(P<0.05).The nursing satisfaction of group Ⅱ(93.33%)was higher than that of group Ⅰ(73.33%),the difference was statistically significant(P<0.05).Conclusion In maintenance hemodialysis patients receiving HD tandem HP treatment,the implementation of comprehensive nursing mode,compared with conventional nursing,can better improve the nutritional status,improve the treatment compliance of patients,as well as the nursing satisfaction of patients and their families,worthy of promotion.
