Expression of PD-1 and TIM-1 in non-small cell lung cancer and their effects on prognosis

摘要 目的探讨非小细胞肺癌(NSCLC)组织中程序性死亡受体-1(PD-1)、T淋巴细胞免疫球蛋白和黏蛋白结构域分子1(TIM-1)的表达及二者对预后的影响。方法选取我院收治的125例NSCLC患者作为研究对象,分别取癌组织及瘤旁组织(距离病灶3~5 cm)以检测PD-1、TIM-1的表达情况。采用Spearman相关系数分析PD-1与TIM-1表达的相关性,并分析二者与临床病理特征的关系。随访24个月,记录患者的生存与死亡情况,采用多因素Cox回归分析影响预后的危险因素,观察不同PD-1、TIM-1表达患者的无瘤生存率与总生存率。结果癌组织中PD-1、TIM-1高表达率高于癌旁组织(P<0.05),Spearman相关系数分析显示PD-1与TIM-1表达呈正相关(r=0.745,P<0.001)。PD-1、TIM-1高表达组临床分期Ⅲ期、中低分化、淋巴结转移、肿瘤直径≥3 cm的比例高于PD-1、TIM-1低表达组(P<0.05)。多因素Cox回归分析显示,临床分期Ⅲ期、中低分化、淋巴结转移、肿瘤直径≥3 cm及PD-1、TIM-1高表达是影响预后的危险因素(P<0.05)。随访期间,125例NSCLC患者中死亡34例(27.20%),生存91例(72.80%),PD-1、TIM-1高表达组的无瘤生存率、总生存率均低于PD-1、TIM-1低表达组(P<0.001)。结论NSCLC患者PD-1、TIM-1以高表达为主,二者高表达会降低患者无瘤生存率与总生存率。 Objective To investigate the expression of programmed death receptor-1(PD-1)and T cell immunoglobulin and mucin domain-containing molecule-1(TIM-1)in non-small cell lung cancer(NSCLC)and their effects on prognosis.Methods A total of 125 patients with NSCLC treated in our hospital were selected as the research objects,and the cancer tissues and adjacent tissues(3 to 5 cm away from the focus)were taken to detect the expression of PD-1 and TIM-1.Spearman correlation coefficient was used to analyze the correlation between PD-1 and TIM-1 expression,and their relationships with clinicopathological features were analyzed.The patients were followed up for 24 months,and their survival and death were recorded.The risk factors affecting the prognosis were analyzed by multivariate Cox regression analysis,and the tumor-free survival rate and overall survival rate of patients with different PD-1 and TIM-1 expression were observed.ResultsThe high expression rate of PD-1 and TIM-1 in cancer tissues was higher than that in adjacent tissues(P<0.05).Spearman correlation coefficient analysis showed that PD-1 was positively correlated with TIM-1 expression(r=0.745,P<0.001).The proportions of clinical stageⅢ,medium and low differentiation,lymph node metastasis and tumor diameter≥3 cm in the PD-1 and TIM-1 high expression groups were higher than those in the PD-1 and TIM-1 low expression groups(P<0.05).Multivariate Cox regression analysis showed that clinical stageⅢ,medium and low differentiation,lymph node metastasis,tumor diameter≥3 cm and high expression of PD-1 and TIM-1 were the risk factors affecting the prognosis(P<0.05).During follow-up,among the 125 patients with NSCLC,34 cases(27.20%)died and 91 cases(72.80%)survived.The tumor-free survival rate and overall survival rate of the PD-1 and TIM-1 high expression groups were lower than those of the PD-1 and TIM-1 low expression groups(P<0.001).Conclusion PD-1 and TIM-1 are mainly highly expressed in NSCLC patients,and high expression of both will reduce the tumo-free survival rate and overall survival rate of patients.
