围产期疼痛—产后抑郁的共病研究及进展

Progress on Comorbidity of Perinatal Pain and Postpartum Depression

近年来,产后抑郁(postpartum depression,PPD)发病率逐年增加,造成沉重的社会负担。研究证实,慢性疼痛与抑郁之间存在高共病性和双向调节关系,而围产期疼痛与PPD的共病关系最近几年才引起关注,推测二者可能由相同的病理生理机制驱动。同时,有效的围产期疼痛管理(如分娩镇痛)可降低PPD的发生率。因此,围产期疼痛-PPD的共病研究为PPD的防治带来了新的曙光。该文从疼痛-抑郁共病角度,讨论围产期疼痛与PPD发生发展的共病关系以及分娩镇痛的积极作用,以期从围产期疼痛管理角度为PPD的防治策略提供新思路。

In recent years,the incidence of postpartum depression(PPD)has increased year by year,and PPD has resulted in heavy social burden.It has been confirmed that there is strong comorbidity and bidirectional relationship between chronic pain and depression,while the comorbidity between perinatal pain and PPD has attracted attention in recent years.Perinatal pain and PPD may be driven by the same pathophysiologic mechanism.At the same time,effective perinatal pain management(such as labor analgesia)may reduce the incidence of PPD.Therefore,the study of perinatal pain-PPD comorbidity will shed new light on the prevention and treatment of PPD.Thus,based on the pain-depression comorbidity,this article focuses on the comorbidity between perinatal pain and PPD,as well as the benefits of labor analgesia,so that it can provide a new idea for prevention and treatment of PPD from the perspective of perinatal pain management.
