摘要
目的分析在有核红细胞检测中应用全自动血液细胞分析仪的临床价值。方法选取2019年2月至2021年2月在新安县第二人民医院行有核红细胞检测的84例受检者为研究对象,收集其外周血液标本依次采用显微镜检查法和全自动血液细胞分析仪进行检测,以显微镜检查法为金标准,比较两种不同检测方式的计数结果,采用线性回归分析两种检测方式间的相关性,分别从批内精密度、稳定性等方面观察全自动血液细胞分析仪的应用价值。结果全自动血液分析仪检出阳性标本占比54.76%,阴性标本占比45.24%;显微镜镜检法检出阳性标本占比58.33%,阴性标本占比41.67%;两种检测方式对比,差异无统计学意义(P>0.05)。以显微镜镜检法为金标准,全自动血液分析仪诊断特异度97.14%、灵敏度91.84%、阳性预测值97.83%、阴性预测值89.47%、准确度94.05%。对45例阳性标本测定值进行线性回归分析显示,显微镜镜检法与全自动血液分析仪测定值之间存在线性关系,得出回归方程Y=0.7483X+1.8191,差异有统计学意义(R^(2)=0.7618,P<0.05)。全自动血液分析仪测定低值标本、中值标本和高值标本有核红细胞批内变异系数值(3.059%、3.429%、3.711%)均小于性能要求变异系数(5%),测定结果合格。全自动血液分析仪测定低值标本、中值标本和高值标本的有核红细胞值无明显变化,批内变异系数(1.685%、1.421%和2.007%)均低于性能要求变异系数(5%),测定结果稳定较好。结论全自动血液细胞分析仪在有核红细胞检测具有性能良好,精密度高、稳定性强及可重复操作等优势,并且其测定结果与显微镜镜检法相比具有良好的线性关系,在保障检测结果准确可靠方面效果突出,临床应用价值显著。
Objective To analyze the clinical value of automatic blood cell analyzer in the detection of nucleated red cells.Methods A total of 84 subjects who underwent nuclear red cell detection in The Second People's Hospital of Xin'an County from February 2019 to February 2021 were selected as the research subjects.Their peripheral blood samples were collected for detection by microscopy and automatic blood cell analyzer successively,with microscopy as the gold standard.The counting results of two different detection methods were compared,the correlation between the two detection methods was analyzed by linear regression,and the application value of automatic blood cell analyzer was observed from the aspects of in-batch precision and stability.Results The percentage of positive and negative specimens detected by automatic hematology analyzer was 54.76%and 45.24%respectively.The proportion of positive and negative specimens detected by microscopy was 58.33%and 41.67%,respectively.There was no significant difference between the two detection methods(P>0.05).The diagnostic specificity,sensitivity,positive predictive value,negative predictive value and accuracy of the automatic hematology analyzer were 97.14%,91.84%,97.83%,89.47%and 94.05%,respectively,with microscopic examination as the gold standard.Linear regression analysis of the values of 45 positive specimens showed that there was a linear relationship between the values of microscopic examination and automatic hematology analyzer,the regression equation Y=0.7483x+1.8191,the difference was statistically significant(R^(2)=0.7618,P<0.05).The coefficient of variation(3.059%,3.429%,3.711%)in the batch of nucleated red cells of low value,median value and high value samples determined by automatic hematology analyzer were all lower than the coefficient of variation(5%)required by performance,and the results were qualified.The values of nucleated red cells in low-value,median and high-value specimens determined by automatic hematology analyzer did not change significantly,and the intra-batch coefficient of variation(1.685%,1.421%and 2.007%)were all lower than the performance requirement coefficient of variation(5%),and the determination results were stable.Conclusion The automatic blood cell analyzer has the advantages of good performance,high precision,strong stability and repeatable operation in the detection of nucleated red cells,and its determination results have a good linear relationship compared with the microscopy method,which has outstanding effect in ensuring the accuracy and reliability of the detection results,and has significant clinical application value.
