D二聚体、降钙素对新生儿呼吸窘迫综合并发呼吸机相关性肺预后评估的价值

The Value of D-dimer and Calcitonin in Evaluating the Prognosis of Neonatal Respiratory Distress Syndrome Complicated with Ventilator-Associated Lung Disease

摘要 目的探讨二聚体(D-D)、降钙素(PCT)对新生儿呼吸窘迫综(RDS)合并发呼吸机相关性肺(VAP)预后评估的价值。方法以100例RDS患儿为对象,根据患者VAP发生情况将患者分为VAP组(n=20)和非VAP组(n=80)。检测D-D、PCT水平,记录VAP患儿预后情况,分析二者与VAP患儿预后的关系。结果 VAP组D-D、PCT水平高于非VAP组(P<0.05)。低危组、中危组、高危组之间亦有差距。20例VAP患者中生存15例(75.00%),死亡5例(25.00%)。死亡组D-D、PCT水平高于生存组(P<0.05)。结论 RDS并发VAP患儿D-D、PCT水平升高,检测二者水平可评估患儿的严重程度及预后情况。 Objective To explore the value of polymer( D-D) and calcitonin( PCT) in the prognosis assessment of neonatal respiratory distress complex( RDS) combined with ventilator-associated lung( VAP).Methods The patients were divided into VAP groups( n-20) and non-VAP groups( n-80) according to the occurrence of VAP in 100 cases. Test D-D and PCT levels,record the prognosis of children with VAP,and analyze the relationship between the two and the prognosis of children with VAP. Results The level of VAP group D-D and PCT was higher than that of non-VAP group( P < 0. 05). There are also gaps between low-risk groups,medium-risk groups and high-risk groups. Of the 20 VAP patients,15( 75. 00%) survived and 5( 25. 00%) died. The level of death group D-D and PCT was higher than that of survival group( P < 0. 05).Conclusion The level of D-D and PCT in children with RDS concurrent VAP was raised,and the detection of both levels could assess the severity and prognosis of the children.
