沙丁胺醇对支气管哮喘胸背部肌筋膜疼痛触发点的影响 被引量:2

摘要 目的探究沙丁胺醇对胸背部肌筋膜疼痛伴有支气管哮喘患者疼痛部位触发点内的交感-感觉耦连作用。方法选取124例胸背部疼痛伴支气管哮喘患者,检测吸入沙丁胺醇前后冈下肌的触发点和压痛点的压痛阈值(PPT),对比检测吸入沙丁胺醇前后正常胫前肌的PPT。比较吸入沙丁胺醇后触发点施以吸入前同样的PPT、牵涉痛域值(PTRP)的视觉模拟评分(VAS)。结果沙丁胺醇吸入后与吸入前比较,触发点与压痛点的PPT均显著增高(P<0.001);沙丁胺醇吸入前、后胫前肌PPT比较差异无统计学意义(P>0.05);沙丁胺醇吸入后触发点施以吸入前同样的PPT、PTRP的VAS明显低于吸入前,差异有统计学意义(P<0.001)。结论交感-感觉耦连作用能改善胸背部肌筋膜疼痛伴支气管哮喘患者的机械致敏和疼痛部位及牵涉性肌肉痛痛觉敏化。 Objective To explore the sympathetic-sensory coupling effect of salbutamol on pain trigger points in patients with chest and back myofascial pain associated with bronchial asthma.Methods A total of 124 cases of patients with bronchial asthma with back pain were selected,infraspinatus trigger point and pressure point tenderness threshold(PPT)were test before and after inhalation of salbutamol,Visual Analogue Scores(VAS)scores of trigger point after inhalation of salbutamol were compared after exposure to the same PPT and PTRP before inhalation.Results PPT of trigger points and tenderness points were significantly increased after inhalation of salbutamol than before(P<0.001).There was no significant difference in PPT of the tibialis anterior muscle before and after salbutamol inhalation(P>0.05).After inhalation of salbutamol,the VAS of the trigger point which exposure to the same PPT and PTRP before inhalation was significantly lower than that before inhalation,and the difference was statistically significant(P<0.001).Conclusion Sympathetic-sensory coupling improves mechanical sensitization and pain site and referred muscle pain sensitization in patients with chest and back myofascial pain with bronchial asthma.
