会阴超声替代传统阴道指检监测产程进展的可行性分析

Feasibility analysis of perineal ultrasound replacing traditional vaginal finger examination in monitoring the progress of labor

目的研究会阴超声替代传统阴道指检监测产程进展的可行性。方法选取2019年5月至2020年12月在该院分娩的103例孕妇为研究对象,使用会阴超声测量宫口扩张大小、胎头进展角度(AOP)、胎头与会阴间的距离(HPD)、大脑中线角度(MLA),进行线性回归分析,判断会阴超声替代传统阴道指检监测产程进展的可行性。结果会阴超声检查宫口扩张大小与阴道指检检查宫口扩张大小呈正相关(B=0.996,t=281.297,P<0.01),AOP(°)与胎先露位置呈正相关(B=0.082,t=22.059,P<0.01),HPD(cm)、MLA(°)与胎先露位置呈负相关(HPD:B=-0.105,t=-4.183,P<0.01;MLA:B=-0.020,t=-5.306,P<0.01)。阴道指检与会阴超声检查满意率差异有统计学意义(P<0.01)。结论会阴超声监测产程进展具有可行性,且满意率优于传统阴道指检。

Objective To study the feasibility of perineal ultrasound in replacement of traditional vaginal finger examination to monitor the progress of labor.Methods A total of 103 case of pregnant women who delivered in this hospital from May 2019 to December 2020 were selected.The size of uterine dilatation,angle of fetal head progress(AOP),head-perineum distance(HPD)and midline angle of brain(MLA)were measured by perineal ultrasound,and linear regression analysis was conducted to determine the feasibility of perineal ultrasound in replacement of traditional vaginal finger examination in monitoring the progress of labor.Results There was a positive relationship between uterine dilation measured by ultrasound and vaginal finger examination(B=0.996,t=281.297,P<0.01),and there was a positive correlation between AOP(°)and fetal presentation position(B=0.082,t=22.059,P<0.01).There was a negative correlation between HPD(cm)and MLA(°)and fetal presentation position[HPD:B=-0.105,t=-4.183,P<0.01;MLA:B=-0.020,t=-5.306,P<0.01].There was significant difference in satisfaction rate between the vaginal finger examination group and the ultrasound group(P<0.01).Conclusion The clinical evaluation of perineal ultrasound is better than that of traditional vaginal finger examination in feasibility and satisfaction rate.
