Expressions of SDF-1α/CXCR7 Pathway Proteins in NSCLC Tissues and Their Relationships with Postoperative Recurrence

摘要 目的检测非小细胞肺癌(non-small cell lung cancer,NSCLC)组织间质细胞衍生因子-1α(stromal derived factor 1α,SDF-1α)/趋化因子CXC受体7(chemokine CXC receptor 7,CXCR7)通路蛋白表达,并分析其与术后复发的关系。方法选取我院2018年3月至2020年6月收治的102例拟行切除术的NSCLC患者进行前瞻性试验,手术后取癌组织和切缘正常组织,采用免疫组化法检测SDF-1α、CXCR7、P-糖蛋白(P-glycoprotein,P-gp)蛋白表达。比较癌组织和切缘正常组织上述蛋白阳性表达率;比较不同临床病理特征患者癌组织上述蛋白阳性表达率;分析NSCLC患者术后1年内复发的危险因素。结果癌组织SDF-1α、CXCR7、P-gp蛋白阳性表达率均高于切缘正常组织(P<0.001);Ⅱb期及以上、未/低分化、有淋巴结转移患者癌组织SDF-1α、CXCR7、P-gp蛋白阳性表达率均高于Ⅱa期及以下、中/高分化、无淋巴结转移患者(P<0.05);术后1年内患者的复发率为29.17%(28/96),Ⅱb期及以上、未/低分化、有淋巴结转移、癌组织SDF-1α、CXCR7、P-gp蛋白阳性表达、未遵医嘱治疗均是术后1年内复发的危险因素(OR=4.354、7.411、6.349、11.382、8.538、9.766、5.427,均为P<0.05)。结论NSCLC组织中SDF-1α/CXCR7通路被激活,SDF-1α、CXCR7、P-gp表达增强,且与分化程度、临床分期、淋巴结转移有关,均可增加术后复发的风险。 Objective To detect the expressions of stromal cell-derived factor-1α(SDF-1α)/chemokine CXC receptor 7(CXCR7)in non-small cell lung cancer(NSCLC)tissues and their relationships with postoperative recurrence.Methods A prospective trial was conducted on 102 patients with NSCLC who were treated in this hospital from March 2018 to June 2020.The tumor tissues and normal tissues at the cutting edge were taken to detect the expressions of SDF-1α,CXCR7 and P-glycoprotein(P-gp)by immunohistochemistry.The positive expression rates of the above proteins were compared between the two tissues.The positive expression rates of the above proteins in cancer tissues of patients with different clinic-pathological features were compared as well.The risk factors of postoperative recurrence within one year were analyzed by logistic multiple regression analysis.Results The positive expression rates of SDF-1α,CXCR7 and P-gp in cancer tissues were higher than that of the normal tissues(P<0.001).And the tissues of undifferentiated or poorly differentiation,≥stageⅡB and lymph node metastasis patients were higher than that of moderate/high differentiation,≤ⅡA stage and no lymph node metastasis(P<0.05).The 1-year recurrence rate was 29.17%(28/96),and undifferentiated or poorly differentiation,≥stageⅡB and lymph node metastasis,positive expressions of SDF-1α,CXCR7 and P-gp in cancer tissues,failure to follow the doctor's advice were the risk factors(OR=4.354,7.411,6.349,11.382,8.538,9.766,5.427 respectively,with all P<0.05).ConclusionSDF-1α/CXCR7 pathway is activated in NSCLC,and the expressions of SDF-1α,CXCR7 and P-gp increase in NSCLC tissues,which are related to the degree of differentiation,clinical stage and lymph node metastasis,can influence the postoperative recurrence.
