经皮肾镜手术患者术后并发尿脓毒血症的危险因素

Risk factors of urinary sepsis after percutaneous nephroscopy

目的探讨经皮肾镜手术患者术后并发尿脓毒血症的危险因素。方法选择2018年1月—2021年1月收治的经皮肾镜手术患者120例,统计术后并发尿脓毒血症患者例数,采用单因素分析、多因素Logistic回归分析筛选术后并发尿脓毒血症的危险因素。结果调查结果显示,术后并发尿脓毒血症的经皮肾镜手术患者共32例,单因素分析,经皮肾镜手术患者术后并发尿脓毒血症的影响因素有性别、年龄、合并糖尿病、尿常规、尿培养、结石直径大小、结石形态、肾积水、术中冲洗量,差异有统计学意义(P<0.05);多因素Logistic回归分析,经皮肾镜手术患者术后并发尿脓毒血症的独立危险因素有女性、年龄≥60岁、尿常规阳性、尿培养阳性、结石直径≥2 cm、中重度肾积水。结论经皮肾镜手术患者术后有尿脓毒血症并发风险,且危险因素涉及多方面,临床应加强术后病情监测,针对高危患者采取相应护理对策。

Objective To investigate the risk factors of urinary sepsis after percutaneous nephroscopy.Methods A total of 120 patients underwent percutaneous nephroscopy from January 2018 to January 2021 were selected.The number of patients with postoperative urinary sepsis was counted,the risk factors of postoperative urinary sepsis were screened by univariate analysis and multivariate Logistic regression analysis.Results According to the survey,by the urine of sepsis after percutaneous nephroscope surgery patients a total of 32 cases,single factor analysis,percutaneous nephroscope surgery patients the influence factors of postoperative urinary sepsis with gender,age,diabetes mellitus,routine urine,urine culture,diameter size,stones form,kidney seeper,intraoperative irrigation quantity(P<0.05);Multivariate Logistic regression analysis showed that the independent risk factors for postoperative urinary sepsis in patients undergoing percutaneous nephroscopy were female,age≥60 years old,positive urine routine,positive urine culture,stone diameter≥2 cm and moderate and severe hydronephrosis.Conclusion Patients underwent percutaneous nephroscopy had the risk of urinary sepsis after operation,and the risk factors involve many aspects.It is necessary to strengthen postoperative monitoring and take corresponding nursing measures for high-risk patients.
