慢性精神分裂症患者病情严重程度对自杀意念的影响

The Severity of Schizophrenia Affects Suicidal Ideation in Patients with Chronic Schizophrenia

目的:调查和分析慢性精神分裂症患者病情严重程度对自杀意念的影响,从而预防自杀相关行为的发生。方法:于2018年1月-2019年12月抽取全国18家精神病院的精神分裂症住院患者作为研究对象,采用自编社会人口学信息问卷、Beck自杀意念量表、阳性和阴性综合征量表(PANSS)对患者社会人口学信息、自杀意念、病情严重程度等情况进行调查和评估。采用单因素分析及二元logistic回归分析病情严重程度对患者自杀意念的影响。结果:慢性精神分裂症患者的自杀意念的发生率约为24.3%。肥胖(BMI≥28.0 kg/m^(2))、大学及以上文化程度及PANSS一般精神症状得分是慢性精神分裂症患者自杀意念的危险因素(OR值分别为1.620、1.732、和1.023,P值均<0.05)。结论:应加强对患者的早期治疗,防止病情恶化,同时控制患者的体重,预防肥胖对于预防自杀意念具有积极作用。

OBJECTIVE To Investigate and analyze the impact of illness severity on suicidal ideation in chronic schizophrenia patients,and to prevent the occurrence of suicide-related behaviors.METHODS The study was a case series study.Participants were selected from 18 psychiatric hospitals across China from January 2018 to December 2019.The social-demographic variables,suicidal ideation and severity of disease were investigated by the Beck Suicidal Ideation Scale,Positive and Negative syndrome Scale(PANSS).The univariate analysis and binary logistic regression were used to analyze the effects of schizophrenia severity on suicidal ideation.RESULTS The incidence of suicidal ideation in patients with chronic schizophrenia was 24.3%.Obesity(BMI≥28.0 kg/m^(2)),college or above education and general psychopathology subscore of PANSS were the risk factors for suicidal ideation in patients with chronic schizophrenia(OR=1.620、1.732、1.023,P<0.05).CONCLIUSION Early appropriate treatment was essential for schizophrenia patients to prevent suicidal ideation.In addition,controlling body weight and obesity prevention had a positive effect on the prevention of suicidal ideation.
