仰卧位旋转拔伸手法治疗神经根型颈椎病临床疗效观察

Clinical Observation on the Treatment of Cervical Spondylotic Radiculopathy with Supine Rotation and Stretching Manipulation

目的:观察仰卧位旋转拔伸治疗手法治疗神经根型颈椎病的临床疗效。方法:选取100例神经根型颈椎病患者,将其随机分为治疗组(n=50)和对照组(n=50)。对照组患者采用坐位牵引手法治疗,治疗组患者采取仰卧位拔伸法治疗,治疗1个疗程后比较临床疗效。结果:(1)治疗后两组患者自觉性主观疼痛评分均有所降低,治疗组疼痛评分降低程度高于对照组(t=6.760,P<0.01)。(2)治疗后治疗组患者压顶试验引发疼痛评分的下降更为明显(t=7.565,P<0.01)。(3)治疗后,治疗组患者VAS评分下降更为明显(t=9.290,P<0.01)。(4)治疗后两组患者椎间盘高度差值比较差异有统计学意义(t=6.240,P<0.01)。结论:对于神经根型颈椎病患者采用仰卧位旋转拔伸手法以及坐位屈颈牵引法进行治疗后,两种方法对患者的症状均有不同程度的改善,其中仰卧位旋转拔伸手法的效果相较于坐位屈颈牵引法的治疗效果更好,在降低患者的疼痛程度的同时可以改善患者四肢放射性麻木、无力等临床症状的发生程度,减轻颈椎对于神经根的压迫程度,临床疗效显著。

OBJECTIVE To observe the clinical effect of supine rotating and stretching manipulation in the treatment of cervical spondylotic radiculopathy. METHODS 100 patients with cervical spondylotic radiculopathy were randomly divided into treatment group and control group( 50 cases each). The control group was treated by sitting flexion and neck traction, and the treatment group was treated by supine rotation and stretching manipulation. After treatment, the effect of two groups were analyzed. RESULTS( 1) The subjective pain( pain of neck, occipital, upper limb and shoulder) of the two groups were compared before and after treatment. There was significant difference between the two groups after one course of treatment(t=6.760,P<0.01).( 2) There was a significant difference between the two groups in pain grading caused by pressing test after one course of treatment(t=7.565,P<0.01).( 3) VAS score comparison: there was a statistical difference in VAS score between the two groups after one course of treatment(t=9.290,P<0.01).( 4) Intervertebral disc height difference compared two groups of treatment after a period of treatment in patients with statistical difference. Intervertebral disc edge height difference before and after comparison: two groups of patients after one course of no statistical difference(t=6.240,P<0.01). CONCLUSION Supine rotation and stretching manipulation and sitting flexion and traction for the treatment of cervical spondylotic radiculopathy could effectively relieve neck, shoulder and back pain, upper limbs and fingers radiation pain, numbness and weakness, improve the blood supply of the affected part, promote the absorption of inflammatory reactants, relieve tissue contracture, release adhesion, relieve or relieve the pressure on nerve roots, but supine rotating and stretching manipulation could effectively relieve the pain, reduce the neck injury and improve the quality of life of patients in the treatment of cervical spondylotic radiculopathy.
