消化内镜治疗非静脉曲张性上消化道出血患者的疗效分析

Analysis of the Curative Effect of Digestive Endoscopy in the Treatment of Patients with Non-varices Upper Gastrointestinal Bleeding

目的探讨消化内镜治疗非静脉曲张性上消化道出血患者的疗效。方法选取2019年1月—2020年12月在该院消化内科治疗的非静脉曲张性上消化道出血患者106例,随机分为两组。对照组使用常规内科治疗,观察组在此基础上使用消化内镜止血治疗,对比两组的临床疗效、止血相关指标、炎性因子水平变化、并发症发生率。结果观察组治疗有效率为96.23%,明显高于对照组的75.47%,差异有统计学意义(χ^(2)=9.368,P<0.05);观察组出血停止时间、肠鸣音恢复时间、住院时间明显短于对照组,再出血率明显低于对照组,差异有统计学意义(P<0.05);观察组治疗后TNF-α、IL-8水平明显低于对照组,差异有统计学意义(P<0.05);观察组感染、穿孔、周围循环障碍等并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论消化内镜治疗非静脉曲张性上消化道出血患者的疗效显著,能提升止血效果,加快止血速度,促进炎症消退,降低再出血率及并发症发生率,具有积极的临床意义。

Objective To investigate the efficacy of digestive endoscopy in the treatment of patients with non-varices upper gastrointestinal bleeding.Methods From January 2019 to December 2020,106 patients with non-varices upper gastrointestinal hemorrhage treated in the hospital's gastroenterology department were selected and randomly divided into two groups.The control group received conventional medical treatment.On this basis,the observation group used digestive endoscopic hemostasis therapy to compare the clinical efficacy,hemostasis-related indicators,changes in inflammatory factor levels,and the incidence of complications between the two groups.Results The effective rate of treatment in the observation group was 96.23%,which was significantly higher than the 75.47%in the control group,the difference was statistically significant(χ^(2)=9.368,P<0.05).The bleeding stop time,bowel sound recovery time,and hospitalization time of the observation group were significantly shorter than those of the control group,and the rebleeding rate was significantly lower than that of the control group,the difference was statistically significant(P<0.05).The levels of TNF-αand IL-8 in the observation group were significantly lower than those in the control group after treatment,t he difference was statistically significant(P<0.05).The incidence of complications such as infection,perforation and peripheral circulatory disturbance in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Gastrointestinal endoscopy is effective in treating patients with non-varices upper gastrointestinal bleeding.It can improve hemostasis,speed up hemostasis,promote inflammation subsidence,reduce rebleeding rate and complication rate,and has positive clinical significance.
