开窗引流术治疗口腔颌面囊肿及囊性病变的效果分析

Analysis of the Effect of Fenestration and Drainage in the Treatment of Oral and Maxillofacial Cysts and Cystic Lesions

目的探讨开窗引流术治疗口腔颌面囊肿及囊性病变的效果。方法选择该院于2020年1月—2021年1月收治的口腔颌面囊肿及囊性病变患者69例,以随机数字法分为观察组(n=34)和对照组(n=35)。对照组采用开放手术治疗,观察组采用开窗引流术治疗,对比疗效。结果观察组治疗效果高于对照组,但差异无统计学意义(P>0.05),术后观察组患者囊腔体积减少率及囊腔面积减少率在2、4、6个月均高,差异有统计学意义(P<0.05);观察组手术时间(41.35±4.97)min、住院时间(5.62±0.87)d均短于对照组,差异有统计学意义(t=14.300、4.634,P<0.05),术中出血量(12.36±5.17)mL少于对照组,差异有统计学意义(t=23.875,P<0.05),术后6个月囊肿复发率观察组低于对照组,但差异无统计学意义(P>0.05);观察组术后6个月含牙囊肿、始基囊肿的治疗有效率均为100.00%,根尖囊肿的治疗有效率最低为85.71%,牙源性角化囊性瘤的治疗有效率为91.67%。结论针对口腔颌面囊肿以及囊性病变患者给予开窗引流术治疗的创伤较小,对患者颌骨形态的保护及恢复效果更佳。

Objective To investigate the effect of fenestration and drainage in the treatment of oral and maxillofacial cysts and cystic lesions.Methods 69 patients with oral and maxillofacial cysts and cystic lesions admitted to the hospital from January 2020 to January 2021 were randomly divided into observation group(n=34)and control group(n=35).The control group was treated with open surgery.The observation group was treated with fenestration drainage and compared the curative effect.Results The treatment effect in the observation group was higher than that in the control group,but the difference was not statistically significant(P>0.05).The reduction rates of lumen volume and lumen area in the observation group were higher at 2,4 and 6 months after operation,and the difference was statistically significant(P<0.05).The operation time(41.35±4.97)min and hospital stay(5.62±0.87)d in the observation group were shorter than those in the control group,the differences were statistically significant(t=14.300,4.634,P<0.05),and the intraoperative blood loss(12.36±5.17)mL was less than those in the control group,the differences were statistically significant(t=23.875,P<0.05),the recurrence rate of cysts in observation group was lower than that in control group 6 months after surgery,but there was not statistically significant(P>0.05).In the observation group,the effective rate was 100.00%for dental cysts and primal cysts,85.71%for apical cysts and 91.67%for odontogenic keratocysts 6 months after operation.Conclusion For patients with oral and maxillofacial cysts and cystic lesions,fenestration and drainage has less trauma,and it has a better protection and recovery effect on the jaw bone shape of the patients.
