全肩关节镜与关节镜下小切口手术在肩袖损伤中的应用及对关节功能的影响

Application of Total Shoulder Arthroscopy and Arthroscopic Small Incision Surgery in Rotator Cuff Injury and Its Influence on Joint Function

目的比较全肩关节镜与关节镜下小切口手术在肩袖损伤患者中的应用效果及对关节功能的影响。方法便利选择2019年2月—2021年1月该院收治的肩袖损伤患者102例,以随机数表法将其划分为两组,各51例。对照组应用关节镜下小切口手术治疗,研究组应用全肩关节镜手术治疗。比较两组患者的临床疗效、手术时间、术中出血量、住院时间、术前与术后6个月时美国加州大学肩关节功能评分(UCLA),以及术后并发症情况。结果研究组治疗总有效率96.08%与对照组94.12%对比,差异无统计学意义(χ^(2)=0.000,P>0.05)。研究组术中出血量(25.65±2.06)mL较对照组(41.65±3.03)mL低,差异有统计学意义(t=31.186,P<0.05);研究组手术时间(90.65±13.03)min、住院时间(5.95±0.58)d与对照组(88.65±10.78)min、(6.12±0.76)d对比,差异无统计学意义(t=0.845、1.270,P>0.05)。术后6个月时,研究组UCLA评分(25.65±2.85)分较对照组(22.65±2.06)分高,差异有统计学意义(t=6.092,P<0.05)。两组术后均未见并发症。结论全肩关节镜与关节镜下小切口手术对肩袖损伤的近期疗效相当,其中关节镜下小切口手术时间较短,全肩关节镜对于患者关节功能的改善效果更为理想,且术中出血量少。

Objective To compare the application effect of total shoulder arthroscopy and arthroscopic small incision surgery in patients with rotator cuff injury and the effect on joint function.Methods Conveniently selected 102 patients with rotator cuff injury admitted to the hospital from February 2019 to January 2021,and divided them into two groups with a random number table,with 51 cases in each.The control group was treated with arthroscopic small incision surgery,and the study group was treated with total shoulder arthroscopic surgery.The clinical efficacy,operative time,intraoperative blood loss,length of hospital stay,the university of california shoulder function score(UCLA)before and 6 months after surgery,and postoperative complications were compared between the two groups.Results The total effective rate of treatment in the study group was 96.08% compared with 94.12% in the control group,the difference was not statistically significant(χ^(2)=0.000,P>0.05).The intraoperative blood loss(25.65±2.06)mL in the study group was lower than that in the control group(41.65±3.03)mL,the difference was statistically significant(t=31.186,P<0.05).There was no statistically significant difference in the operation time(90.65±13.03)min and hospitalization time(5.95±0.58)d between the study group and the control group(88.65±10.78)min and(6.12±0.76)d(t=0.845,1.270,P>0.05).At 6 months after surgery,the UCLA score of the study group(25.65±2.85)points was higher than that of the control group(22.65±2.06)points,the difference was statistically significant(t=6.092,P<0.05).There was no postoperative complications in the two groups.Conclusion The short-term curative effect of total shoulder arthroscopy and arthroscopic small incision surgery for rotator cuff injury is similar,and the time of arthroscopic small incision surgery is shorter.The total shoulder arthroscopy is more ideal for improving the joint function of patients,and has less intraoperative bleeding.
