着色性干皮病并发巨大鳞状细胞癌

Xeroderma pigmentosum complicated with giant squamous cell carcinoma: a case report

报告1例着色性干皮病并发巨大鳞状细胞癌。患儿男,10岁。因面部褐色斑疹十年并左眼周、鼻部溃疡3年就诊。皮肤科检查:面部针尖到绿豆大褐色、暗棕色色素沉着斑,多发性边缘不规则深在性溃疡,多个菜花状增生物,左睑丧失正常形态,左下睑外翻,结膜充血。组织病理示:真皮内部分浸润细胞核大、深染,可见散在核分裂相,部分区域有角珠形成,考虑鳞状细胞癌。诊断:着色性干皮病并发巨大鳞状细胞癌。

A case of xeroderma pigmentosum complicated with giant squamous cell carcinoma is reported.A 10-year-old boy presented with brown patches on the face for over 10 years,and ulceration around the left eyelid and dorsal nose over 3 years.Physical examination showed brown pigmented spots or patches,sized from needle tip to mung bean,on the face.There were multiple deep ulcers with irregular borders and cauliflower-like masses on the face.Eye examination showed ectropion of left lower eyelid and conjunctival congestion of left eye.Histopathological examination showed atypical keratinocytic proliferation in the dermis.Most atypical infiltrates showed enlarged and hyperchromatic nuclei.Scattered mitoses were noted.Keratin pearls were fbcally seen.The histopathological findings are those of squamous cell carcinoma.The diagnosis of xeroderma pigmentosus complicated with giant squamous cell carcinoma was established.
