《甲状腺结节及相关疾病热消融中西医结合诊疗指南》(2021版)

热消融作为一种针对甲状腺结节及相关疾病新型微创治疗方法,其在甲状腺肿瘤中的疗效已得到证实。但热消融术仍然存在适应证是否合适、临床诊治操作是否规范、术后如何促进康复、中医中药在热消融治疗过程如何发挥作用等问题。为进一步规范甲状腺结节相关疾病的诊治,充分发挥中医中药的优势,提高热消融治疗效果,促进患者康复,由上海市中西医结合学会甲状腺疾病专业委员会牵头联合全国甲状腺疾病中西医结合诊治专家组,依照《世界卫生组织指南制定手册》,综合分析高质量临床证据,汇集国内著名专家的临床经验与意见,经过多次专家研讨,几经易稿,达成共识,制定了“甲状腺结节及相关疾病热消融中西医结合诊疗指南(2021版)”。本指南包括甲状腺良性结节(包括单纯性甲状腺肿、结节伴慢性淋巴细胞性甲状腺炎、甲状腺高功能腺瘤、甲状腺腺瘤及单纯性甲状腺肿囊性病变)、微小乳头状癌、甲状腺癌热消融术后复发结节及淋巴结转移灶的诊断、分期辨证论治、热消融治疗准则与操作规范、疗效评价及疾病预防与调护。

As an alternative to traditional surgery,thermal ablation has been proven to be effective in thyroid tumors.However,there are still problems such as whether the indications are appropriate,whether the clinical diagnosis and treatment operations are standardized,how to promote rehabilitation after surgery,and how TCM plays a role in the treatment process of thermal ablation.Therefore,in order to further standardize the diagnosis and treatment of thyroid nodulerelated diseases,and at the same time integrate the advantages of traditional Chinese medicine to achieve the characteristics of integrated traditional Chinese medicine and western medicine by combined traditional Chinese medicine with ablation.Thyroid Committee of Shanghai Association of Chinese Integrative Medicine and Thyroid Tumor Ablation Experts Croup of Chinese Medical Doctor Association had organized many seminars,in accordance with the World Health Organization Guideline Development Manual,comprehensive analysis of high-quality clinical evidence,collection of clinical experience and opinions of well-known domestic experts,after many expert discussions and revisions,and finally came to the consensus to formulate the Chinese Guidelines for the Diagnosis and Operational of Combined Thermal Ablation and Traditional Chinese Medicine for Thyroid Nodules and Related Diseases(2021 Edition).This guideline includes the diagnosis of benign thyroid nodules(including simple goiter,nodules with chronic lymphocytic thyroiditis,hyper-functional thyroid adenomas,thyroid adenomas and simple goiter cystic lesions),micro papillary carcinoma,recurrent nodules and lymph node metastases after thermal ablation of thyroid cancer,staging dialectical treatment,heat ablation treatment guidelines and operational norms,efficacy evaluation,and disease prevention and care.
