分析腹腔镜胆囊切除术对胆结石的治疗效果及安全性

Analysis of the Therapeutic Effect and Safety of Laparoscopic Cholecystectomy on Gallstones

目的观察腹腔镜胆囊切除术治疗胆结石的临床效果及安全性。方法选择医院2018年1月—2020年1月接收的胆结石患者65例作为研究对象,依据手术方式分组,即开腹组和腹腔镜组,其中开腹组32例,行开腹胆囊切除术治疗,腹腔镜组33例,行腹腔镜胆囊切除术治疗,观察手术效果、安全性、疼痛程度改善情况以及治疗效果。结果腹腔镜组手术时间、术中出血量、术后排气时间、住院时间均显著低于开腹组,差异有统计学意义(P<0.05);腹腔镜组术后切口感染、腹腔感染、肠粘连等并发症发生率显著低于开腹组,差异有统计学意义(P<0.05);术前,两组VAS评分差异无统计学意义(P>0.05);腹腔镜组术后6h、术后12 h VAS评分显著低于开腹组,差异有统计学意义(P<0.05)。开腹组和腹腔镜组患者分别在实施常规开腹胆囊切除术和腹腔镜胆囊切除术治疗后,开腹组和腹腔镜组治疗总有效率分别为81.25%、96.97%,差异有统计学意义(P<0.05)。结论胆结石应用腹腔镜胆囊切除术治疗时,具有良好的手术效果及安全性,利于患者术后尽早康复,缩短住院时间。

Objective To observe the clinical effect and safety of laparoscopic cholecystectomy in the treatment of gallstones.Methods A total of 65 patients with gallstones in our hospital from January2018 to January 2020 were selected as the research objects,and they were divided into open group and laparoscopic group according to the operation methods,including 32 cases in open group,treated with open cholecystectomy,and 33 cases in laparoscopic group,treated with laparoscopic cholecystectomy.The operation effect,safety,improvement of pain degree and treatment effect were observed.Results The operation time,intraoperative blood loss,postoperative exhaust time and hospital stay of the laparoscopic group were significantly lower than those of the open group,the difference was statistically significant(P<0.05).The incidence of postoperative complications such as incision infection,abdominal infection,intestinal adhesion in laparoscopic group was significantly lower than that in open group,the difference was statistically significant(P<0.05).Before operation,there was no significant difference in VAS score between the two groups(P>0.05).The VAS score of laparoscopic group was significantly lower than that of open group at 6h and 12h after operation,and the difference was statistically significant(P<0.05).After conventional open cholecystectomy and laparoscopic cholecystectomy,the total effective rates of the open and laparoscopic groups were 81.25%and 96.97%,respectively,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic cholecystectomy in the treatment of gallstones has good surgical effect and safety,which is conducive to the recovery of patients as soon as possible and shorten the length of hospital stay.
