甲状腺冰冻切片病理检查在鉴别其良恶性价值中的应用

Application of the Value of Thyroid Frozen Section Pathological Examination in Differentiating Benign From Malignant

目的分析甲状腺冰冻切片病理检查鉴别诊断甲状腺疾病良恶性的价值。方法选取医院2017年12月—2020年12月收治的接受手术治疗的200例甲状腺疾病患者,术中将肿瘤切除后,将肿瘤组织制作成冰冻切片、石蜡切片开展病理检查,对肿瘤良恶性进行鉴别诊断;将肿瘤石蜡切片病理检查结果当成诊断“金标准”,分析甲状腺冰冻切片病理检查的价值。结果冰冻切片病理检查的诊断所需时间为(27.81±3.24)min,明显短于石蜡切片病理检查的(2358.75±602.18)min,比较差异有统计学意义(P<0.05)。石蜡切片病理检查结果显示,全部200例患者中,50例患者为良性肿瘤,105例患者为恶性肿瘤;45例滤泡性肿瘤的良恶性须待常规。在检查诊断甲状腺良性肿瘤、恶性肿瘤时,石蜡切片病理检查的诊断符合率分别为100%(50/50)、97.1%(102/105);因甲状腺滤泡性肿瘤的诊断须待常规石蜡切片故不作诊断符合率统计学分析。结论在鉴别诊断甲状腺疾病良恶性时,采用甲状腺冰冻切片病理检查不仅能让诊断所需时间明显缩短,而且具有较高的诊断准确性,值得临床推广。

Objective To analyze the value of thyroid frozen section in differential diagnosis of benign and malignant thyroid diseases.Methods A total of 200 patients with thyroid diseases who received surgical treatment in our hospital from December 2017 to December 2020 were selected.After the tumor was removed,the tumor tissue was made into frozen section and paraffin section for pathological examination to make differential diagnosis between benign and malignant tumors.Analyzed the value of frozen thyroid section pathological examination,the results of paraffin section pathological examination were regarded as the“gold standard”of diagnosis.Results The diagnosis time of frozen section was(27.81±3.24)min,significantly shorter than(2358.75±602.18)min,the difference was statistically significant(P<0.05).Paraffin section pathological examination results showed that in all 200 patients,50 patients were benign tumors and 105 patients were malignant tumors;45 cases of follicular tumors should be treated routinely.The coincidence rate of paraffin section was 100%(50/50),97.1%(102/105).Because the diagnosis of thyroid follicular tumor needs to wait for routine paraffin section,the diagnostic coincidence rate was not statistically analyzed.Conclusion In the differential diagnosis of benign and malignant thyroid diseases,thyroid frozen section pathological examination can not only significantly shorten the diagnosis time,but also has high diagnostic accuracy,which is worthy of clinical promotion.
