综合护理措施对妊娠期高血压疾病患者疾病控制与妊娠结局的影响

Effect of Comprehensive Nursing Measures on Disease Control and Pregnancy Outcome of Patients With Hypertensive Disorders of Pregnancy

目的探讨综合护理措施对妊娠期高血压疾病(hypertensive disorders of pregnancy,HDP)患者疾病控制与妊娠结局的影响。方法98例研究对象均为2017年8月—2020年7月聊城市东昌府区妇幼保健院收治的HDP患者,将其按照随机数表法分组。对照组49例行常规护理,研究组49例在此基础上应用综合护理措施。在干预前后对比两组患者的血压指标以及妊娠结局(子痫、胎盘早剥、产后出血、剖宫产)与新生儿结局(宫内窘迫、早产、新生儿窒息、新生儿死亡)。结果干预后研究组HDP患者收缩压与舒张压水平较对照组更低,差异有统计学意义(P<0.01)。研究组剖宫产的发生率较对照组更低,差异有统计学意义(P<0.05),两组子痫、胎盘早剥、产后出血的发生率对比差异无统计学意义(P>0.05)。两组新生儿宫内窘迫、早产、新生儿窒息、新生儿死亡的发生率对比差异无统计学意义(P>0.05)。结论综合护理措施可以有效控制HDP患者的血压水平,降低剖宫产概率,适于临床应用与推广。

Objective To explore the effect of comprehensive nursing measures on disease control and pregnancy outcome of patients with hypertensive disorders of pregnancy(HDP).Methods A total of 98 patients with HDP in Maternal and Child Health Hospital,Dongchangfu District from August 2017 to July 2020 were selected.They were randomly divided into two groups by random number method.The control group(49 cases)was received routine nursing,and the study group(49 cases)was received comprehensive nursing on this basis.Before and after the intervention,the blood pressure indexes,pregnancy outcomes(eclampsia,placental abruption,postpartum hemorrhage,cesarean section)and neonatal outcomes(intrauterine distress,preterm birth,neonatal asphyxia,neonatal death)were compared between the two groups.Results After the intervention,the systolic and diastolic blood pressure levels in the study group were lower than those in the control group,the difference was statistically significant(P<0.01).The incidence of cesarean section in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of eclampsia,placental abruption and postpartum hemorrhage between the two groups(P>0.05).There was no significant difference in the incidence of neonatal distress,premature birth,neonatal asphyxia and neonatal death between the two groups(P>0.05).Conclusion Comprehensive nursing measures can effectively control the blood pressure level of patients with hypertensive disorders of pregnancy and reduce the probability of cesarean section,which is suitable for clinical application and promotion.
