COX2、β-catenin在子宫肌瘤组织中的表达及其临床意义

Expressions and clinical significance of COX2 and β-catenin in uterine leiomyoma

目的检测子宫肌瘤患者子宫肌瘤组织中环氧化酶-2(COX2)和β-连环蛋白(β-cat)的表达情况,并探讨二者在子宫肌瘤发病过程中的作用及临床意义。方法回顾性选取2017年6月至2019年12月海南医学院第二附属医院妇科收治的65例子宫肌瘤患者,均行腹腔镜下子宫肌瘤切除术,取其手术切除的子宫肌瘤组织和肌瘤旁正常组织,并将其分为子宫肌瘤组65例和对照组65例。采用实时荧光定量PCR(qRT-PCR)法检测COX2、β-cat mRNA表达水平;分析COX2、β-cat表达水平与子宫肌瘤临床病理特征之间的关系;采用Pearson法分析子宫肌瘤组织中COX2与β-cat表达水平的相关性;并对影响患者子宫肌瘤发生的相关因素进行Logistic回归分析。结果子宫肌瘤组COX2、β-cat mRNA表达水平为1.26±0.35、0.91±0.30,均显著高于对照组(0.75±0.28、0.59±0.18),差异均有统计学意义(P<0.05)。多发性子宫肌瘤患者COX2、β-cat mRNA表达水平为1.30±0.33、0.95±0.31,显著高于单发性子宫肌瘤患者(1.07±0.26、0.74±0.20),绝经患者COX2、β-cat mRNA表达水平为1.35±0.37、0.98±0.33显著高于未绝经患者(1.13±0.25、0.82±0.25),差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,子宫肌瘤组织中COX2与β-cat表达水平呈明显正相关(P<0.05)。COX2高表达、β-cat高表达是影响子宫肌瘤发生的独立危险因素(OR=3.591,95%CI:1.435~8.986;OR=2.935,95%CI:1.286~6.698;P<0.05)。结论子宫肌瘤患者子宫肌瘤组织中COX2、β-cat表达水平明显升高,且与病理类型、绝经密切相关,提示子宫肌瘤组织中COX2、β-cat可作为子宫肌瘤患者病情评估的潜在生物学指标。

Objective To detect the expressions of cyclooxygenase-2(COX-2)andβ-catenin(β-cat)in uterine leiomyoma,and to explore the role and clinical significance of COX-2 andβ-catenin in in the pathogenesis of uterine leiomyoma.Methods From June 2017 to December 2019,65 patients with hysteromyoma treated in the Department of Gynecology,the Second Affiliated Hospital of Hainan Medical University underwent laparoscopic hysteromyomectomy were retrospectively selected.The myoma tissue and normal tissue adjacent to the myoma were taken and divided into uterine myoma group(65 cases)and control group(65 cases).The mRNA expression levels of COX2 andβ-cat were detect ed by real-time fluorescence quantitative PCR(qRT-PCR);the relationship between the expression levels of COX2 andβ-cat and the clinicopathological characteristics of uterine leiomyoma was analyzed.The correlation between COX2 andβ-cat expression levels in uterine fibroids was analyzed using Pearson's method.The related factors affecting the occurrence of uterine fibroids in patients were analyzed by Logistic regression.Results The expression levels of COX-2 andβ-cat mRNA in hysteromyoma group were 1.26±0.35,0.91±0.30,which were significantly higher than those in control group(0.75±0.28,0.59±0.18),the differences were statistically significant(P<0.05).The mRNA expression levels of COX2 andβ-cat in patients with multiple uterine fibroids were 1.30±0.33 and 0.95±0.31,which were significantly higher than those in patients with single uterine fibroids(1.07±0.26,0.74±0.20),the mRNA expression levels of COX-2 andβ-cat in postmenopausal patients were 1.35±0.37 and 0.98±0.33,which were significantly higher than those in premenopausal patients(1.13±0.25,0.82±0.25),and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that there was a significant positive correlation between COX-2 andβ-cat(P<0.05).The high expressions of COX-2 andβ-cat were independent risk factors for the occurrence of uterine leiomyoma(OR=3.591,95%CI:1.435-8.986;OR=2.935,95%CI:1.286-6.698;P<0.05).Conclusion The expression levels of COX-2 andβ-cat in uterine leiomyoma tissue are significantly increased,which are closely related to pathological type and menopause.It is suggested that COX-2 andβ-cat in uterine leiomyoma tissue can be used as potential biological indicators for evaluating the condition of uterine leiomyoma patients.
