综合性护理对前置胎盘手术患者术后出血量、并发症和满意度的影响

Effect of comprehensive nursing on postoperative blood loss,complications and satisfaction of patients after surgery for placenta praevia

目的探讨综合性护理对接受前置胎盘手术患者术后出血量、并发症和满意度的影响。方法选取2019年3月至2020年10月期间在南京医科大学附属淮安第一医院产科接受前置胎盘手术的120例患者为研究对象,采用随机数字表法分为照组(n=60)和观察组(n=60),分别接受常规护理和综合护理。比较2组患者的应激反应、术后出血量、住院总时长、并发症发生率和满意度。结果关腹时,2组的收缩压和舒张压均显著高于同组入室时(P均<0.05),心率均显著快于同组入室时(P均<0.05);但观察组的收缩压和舒张压均显著低于对照组(P均<0.05),心率显著慢于对照组(P<0.05)。观察组的术后出血量显著少于对照组(P<0.05),住院总时长显著短于对照组(P<0.05),术后并发症发生率显著低于对照组(P<0.05),满意度各项评分均显著高于对照组(P均<0.05)。结论对行前置胎盘手术的患者实施综合性护理,可有效减轻其术中应激反应,减少其术后出血量,降低其术后并发症发生情况,提高患者的满意度。

Objective To explore the effect of comprehensive nursing on postoperative blood loss,complications and satisfaction of patients after surgery for placenta praevia.Methods A total of120patients who underwent surgery for placenta praevia in the Department of Obstetrics at the Affiliated Huaian No.1People.s Hospital of Nanjing Medical University from March2019to October2020were selected and divided into a control group(n=60)and an observation group(n=60)according to the random number table method.They were given routine nursing and comprehensive nursing respectively.The stress reactions,amount of postoperative blood loss,length of hospital stay,complication rate and satisfaction scores were compared between the two groups.Results When closing the abdomen,the systolic pressure,diastolic pressure and heart rate of the two groups were significantly higher than those when entering the operation room(all P<0.05),but the systolic pressure,diastolic pressure and heart rate of the observation group were significantly lower than those of the control group(all P<0.05).The amount of postoperative blood loss in the observation group was significantly less than that in the control group(P<0.05).The length of hospital stay in the observation group was significantly shorter than that in the control group(P<0.05).The postoperative complication rate was significantly lower than that in the control group(P<0.05).The scores of various items of satisfaction in the observation group were significantly higher than those in the control group(all P<0.05).Conclusion Comprehensive nursing for patients after surgery for placenta praevia can effectively reduce their intraoperative stress reactions and postoperative blood loss,reduce the incidence of postoperative complications and improve their satisfaction.
