基于快速康复理念的围手术期护理对行胸腔镜下肺癌根治术患者术后恢复情况的影响

Effect of perioperative nursing based on the concept of fast-track surgery on postoperative recovery of patients after thoracoscopic radical resection of lung cancer

目的探讨基于快速康复理念的围手术期护理对行胸腔镜下肺癌根治术患者术后恢复情况的影响。方法选取2019年1月至2020年12月在徐州医科大学附属医院接受胸腔镜下肺癌根治术的100例患者,按随机数字表法分为对照组(n=50)和观察组(n=50)。对照组患者接受常规护理,观察组患者在常规护理的基础上接受基于快速康复理念的围手术期护理。比较2组患者的术后恢复指标、护理满意率、生活质量和术后并发症发生率。结果观察组的胸腔闭式引流管拔除时间、下床活动时间和排气时间均显著早于对照组(P均<0.05),抗生素使用时间和住院时间均显著短于对照组(P均<0.05)。观察组的护理满意率(92.00%)显著高于对照组(76.00%,P<0.05)。术后2个月,2组的总体健康、躯体疼痛、精神健康、情感职能和生理职能评分均显著高于同组护理前(P均<0.05),且观察组的上述评分均显著高于对照组(P均<0.05)。观察组的并发症发生率(12.00%)显著低于对照组(28.00%,P<0.05)。结论基于快速康复理念的围手术期护理应用于行胸腔镜下肺癌根治术患者,可有效缩短其恢复进程,减少并发症发生,提高生活质量和护理满意率。

Objective To explore the effect of perioperative nursing based on the concept of fasttrack surgery on postoperative recovery in patients after thoracoscopic radical resection of lung cancer.Methods A total of100patients who underwent thoracoscopic radical resection of lung cancer at the Affiliated Hospital of Xuzhou Medical University from January2019to December2020were selected as research subjects.They were divided into a control group(n=50)and an observation group(n=50)by random number table method.The control group was given routine nursing,while the observation group was given perioperative nursing based on the concept of fast-track surgery on the basis of routine nursing.The postoperative recovery indicators,nursing satisfaction rate,quality of life and postoperative complication rate were compared between the two groups.Results The extubation time of closed thoracic drainage tube,time to get out of bed,and anal exhaust time in the observation group were significantly earlier than those in the control group(all P<0.05).The duration of antibiotic treatment and length of hospital stay in the observation group were significantly shorter than those in the control group(both P<0.05).The nursing satisfaction rate of the observation group(92.00%)was significantly higher than that of the control group(76.00%,P<0.05).Two months after the operation,the scores of general health,physical pain,mental health,emotional function and physiological function of the two groups were significantly higher than those before nursing(all P<0.05),and the above-mentioned scores of the observation group were significantly higher than those of the control group(all P<0.05).The complication rate in the observation group(12.00%)was significantly lower than that in the control group(28.00%,P<0.05).Conclusion Perioperative nursing based on the concept of fast-track surgery for patients after thoracoscopic radical resection of lung cancer can effectively promote their postoperative recovery,reduce the postoperative complication rate,and improve the quality of life and nursing satisfaction rate.
