吴茱萸加粗盐热烫对行腹腔镜手术的泌尿外科患者术后腹胀的效果

Effect of hot compress with Evodia rutaecarpa and coarse salt on postoperative abdominal distension in patients undergoing laparoscopic urological surgery

目的探讨吴茱萸加粗盐热烫对行腹腔镜手术的泌尿外科患者术后腹胀的缓解效果。方法选取南方医科大学顺德医院泌尿外科于2020年1~10月收治的98例行腹腔镜手术后出现腹胀的患者为研究对象,按照随机数字表法分为对照组(n=49)和观察组(n=49)。对照组患者采用常规护理,观察组患者在常规护理的基础上采用吴茱萸加粗盐热烫。比较2组患者的临床疗效、术后首次排气时间、首次排便时间、并发症发生情况、疼痛情况和腹胀程度。结果观察组的总有效率高于对照组,差异有统计学意义(P<0.05)。术后36h和48h,2组的视觉模拟评分法(VAS)评分相较于同组术后24h均显著升高(P均<0.05),但观察组的VAS评分均较同期对照组更低,差异均有统计学意义(P均<0.05)。观察组的首次排气时间和排便时间均显著早于对照组(P均<0.05)。干预后,2组的腹胀(轻度、中度和重度腹胀)发生率均低于同组干预前,且观察组的轻度腹胀发生率低于对照组,差异均有统计学意义(P均<0.05)。观察组和对照组的术后并发症总发生率分别为4.08%和10.20%,观察组略低于对照组,但差异无统计学意义(P>0.05)。结论吴茱萸加粗盐热烫可明显促进行腹腔镜手术的泌尿外科患者术后胃肠道功能的恢复,缓解其腹胀和疼痛程度。

Objective To investigate the effect of hot compress with Evodia rutaecarpa and coarse salt on the relief of postoperative abdominal distension in patients undergoing laparoscopic urological surgery.Methods A total of98patients with abdominal distension after laparoscopic surgery in the Department of Urology at Shunde Hospital of Southern Medical University from January2020to October2020were selected as the research objects.They were divided into a control group(n=49)and an observation group(n=49)by random number table method.The patients in the control group were given routine nursing,and the patients in the observation group received hot compress with Evodia rutaecarpa and coarse salt on the basis of routine nursing.The clinical efficacy,first exhaust time after surgery,first defecation time,incidence of complications,and degree of pain and abdominal distension were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).At36and48hours after surgery,the visual analogue scale(VAS)scores of the two groups were significantly higher than those at24hours after surgery(all P<0.05),The VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(both P<0.05).The first exhaust time and first defecation time of the observation group were significantly earlier than those of the control group(both P<0.05).After the intervention,the incidences of abdominal distension(mild,moderate and severe abdominal distension)in the two groups were lower than those before the intervention,and the incidence of mild abdominal distension in the observation group was lower than that in the control group.The differences were statistically significant(all P<0.05).The total incidence of postoperative complications in the observation group and control group were4.08%and10.20%,respectively.The incidence of the observation group was lower than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion Hot compress with Evodia rutaecarpa and coarse salt can significantly promote the recovery of gastrointestinal function and relieve the abdominal distension and pain in patients undergoing laparoscopic urological surgery.
