关节镜下手术患者术后疼痛的影响因素分析

Analysis of Influencing Factors of Postoperative Pain in Patients Undergoing Arthroscopic Surgery

目的:探讨关节镜下手术患者术后疼痛的影响因素。方法:回顾性分析2020年1月-2021年7月在九江市中医医院行关节镜手术治疗的96例半月板损伤患者的临床资料,统计患者术后疼痛发生情况,分析关节镜下手术患者术后疼痛的影响因素。结果:关节镜下手术患者术后疼痛发生率为84.38%(81/96)。单因素分析显示:年龄、术后负重时间、肢体冷敷、文化程度、关节软骨损伤与关节镜下手术患者术后疼痛发生有关,差异均有统计学意义(P<0.05);体重指数、性别、手术类型、手术范围、ASA分级、滑膜切除与关节镜下手术患者术后疼痛发生无关,差异均无统计学意义(P>0.05)。多因素分析显示:年龄≥60岁[OR=7.857,95%CI(2.256,27.359)]、术后负重时间<7 d[OR=4.286,95%CI(1.362,13.485)]、文化程度初中及以下[OR=4.931,95%CI(1.440,16.891)]、关节软骨损伤[OR=5.500,95%CI(1.601,18.894)]是关节镜下手术患者术后发生疼痛的高危因素(P<0.05),肢体冷敷[OR=0.082,95%CI(0.021,0.320)]是其保护因素(P<0.05)。结论:关节镜下手术患者术后疼痛发生与年龄、术后负重时间、肢体冷敷、文化程度、关节软骨损伤有关。

Objective:To explore the influencing factors of postoperative pain in patients undergoing arthroscopic surgery.Method:The clinical data of 96 patients with meniscus injury who underwent arthroscopic surgery in Jiujiang Hospital of Traditional Chinese Medicine from January 2020 to July 2021 were retrospectively analyzed,the incidence of postoperative pain was counted,the influencing factors of postoperative pain in patients undergoing arthroscopic surgery was analyzed.Result:The incidence of postoperative pain in patients underwent arthroscopic surgery was 84.38% (81/96).Univariate analysis showed that age,postoperative weight-bearing time,cold compress of limbs,education level and articular cartilage injury were related to postoperative pain in patients underwent arthroscopic surgery,the differences were statistically significant (P<0.05);body mass index,gender,surgical type,surgical scope,ASA grade and synovial excision were not related to postoperative pain in patients undergoing arthroscopic surgery,and there were no statistical significance (P>0.05).Multivariate analysis shows that age ≥60 years[OR=7.857,95%CI (2.256,27.359)],postoperative weight-bearing time <7 d[OR=4.286,95%CI (1.362,13.485)],education level junior high school or below[OR=4.931,95%CI (1.440,16.891)],articular cartilage injury[OR=5.500,95%CI (1.601,18.894)]were high risk factors for postoperative pain in patients underwent arthroscopic surgery (P<0.05),cold compress or limbs[OR=0.082,95%CI (0.021,0.320)]was a protective factor (P<0.05).Conclusion:The incidence of postoperative pain in patients underwent arthroscopic surgery is related to age,postoperative weight-bearing time,cold compression of limbs,education level,and articular cartilage injury.
