摘要
目的:探讨超脉冲CO_(2)激光磨削联合负压吸疱自体表皮移植术治疗白癜风的临床效果。方法:选取2019年1月-2021年1月鞍钢集团公司总医院收治的90例白癜风患者,依据随机数字表法分为研究组和对照组,各45例。对照组给予滚轮磨削法治疗,研究组给予超脉冲CO_(2)激光磨削联合负压吸疱自体表皮移植术治疗。比较两组治疗效果,IFN-γ、IL-4指标水平变化情况,皮损面积变化情况,皮损色素积分以及不同疱皮个数磨削时间。结果:治疗前后,两组IL-4水平比较,差异均无统计学意义(P>0.05);与对照组相比,研究组治疗后IFN-γ水平明显较低(P<0.05)。与对照组相比,研究组总有效率明显较高(P<0.05)。治疗前,两组皮损面积及皮损色素积分比较,差异均无统计学意义(P>0.05);与对照组相比,研究组治疗后3、6个月皮损面积均明显较小,且皮损色素积分均相对较高(P<0.05)。与对照组相比,研究组不同疱皮个数磨削时间均明显较短(P<0.05)。结论:白癜风患者接受超脉冲CO_(2)激光磨削联合负压吸疱自体表皮移植术可有效增强治疗效果,缩减皮损面积,并能显著提升皮损色素积分,促进患者炎症因子水平缓解,同时还能使受皮区磨削时间明显缩减。
Objective:To investigate the clinical effect of ultra-pulsed CO_(2)laser grinding combined with negative pressure blister autoepidermal transplantation in the treatment of vitiligo.Method:A total of 90 patients with vitiligo admitted to Ansteel Group General Hospital from January 2019 to January 2021 were selected,they were divided into study group and control group according to random number table method,with 45 cases in each group.The control group was treated with roller grinding,and the study group was treated with ultra-pulsed CO_(2)laser grinding combined with negative pressure blister autoepidermal transplantation.The treatment effect,changes of IFN-γ and IL-4 index levels,changes of skin area,skin pigment integral and grinding time of different number of blister skin were compared between the two groups.Result:Before and after treatment,there were no significant differences in IL-4 levels between the two groups (P>0.05);compared with the control group,IFN-γ level in the study group was significantly lower after treatment (P<0.05).Compared with the control group,the total effective rate in the study group was significantly higher (P<0.05).Before treatment,there were no significant differences in skin area and skin pigment scores between the two groups (P>0.05);compared with the control group,the skin lesion areas of the study group were significantly smaller 3 and 6 months after treatment,and the skin pigment scores were relatively higher (P<0.05).Compared with the control group,the grinding time of different number of blister skin in the study group were significantly shorter (P<0.05).Conclusion:Patients with vitiligo receive ultra-pulsed CO_(2)laser grinding combined with negative pressure blister suction autologous epidermal transplantation,which can effectively enhance the treatment effect,reduce the area of skin lesions,significantly improve the pigment score of skin lesions,promote the remission of the level of inflammatory factors,and significantly reduce the grinding time of the recipient area.
