右佐匹克隆联合奥氮平治疗难治性失眠症疗效及安全性观察

Efficacy of Dexzopiclone Combined with Olanzapine in the Treatment of Refractory Insomnia

目的:探究右佐匹克隆联合奥氮平治疗难治性失眠症的疗效。方法:选取2018年2月至2020年8月淄博市精神卫生中心收治的难治性失眠症患者92例作为研究对象,按照随机数字表法分为观察组与对照组,每组46例。对照组给予右佐匹克隆口服治疗,观察组给予奥氮平右佐匹克隆联合口服治疗。治疗4周后,采用匹兹堡睡眠质量指数评价2组临床治疗效果疗效,采用多导睡眠仪监测2组患者睡眠质量,并比较2组药物不良反应的差异性。结果:经治疗4周后,观察组总有效率95.65%,明显高于对照组67.39%,差异有统计学意义(P<0.05);治疗前,2组睡眠质量比较,差异无统计学意义(P>0.05)。观察组睡眠时间(381.56±75.23)min,睡眠潜伏期(19.24±11.47)min,快速眼球运动潜伏期(151.36±58.24)min,睡眠效率(85.54±6.65)%,优于对照组,差异有统计学意义(P<0.05)。观察组出现恶心2例、呕吐2例、体质量增加1例,总发生率10.87%,低于对照组13.04%,差异有统计学意义(P<0.05)。结论:右佐匹克隆联合奥氮平治疗难治性失眠症可有效提高临床疗效,改善睡眠质量,值得推广。

Objective:To explore the efficacy of dexzopiclone combined with olanzapine in the treatment of refractory insomnia.Methods:A total of 92 patients with refractory insomnia treated in Zibo mental health center from February 2018 to August 2020 were selected as the research object.They were randomly divided into observation group and control group,with 46 cases in each group.The control group was given oral treatment of right zopiclone,and the observation group was given oral treatment of olanzapine and right zopiclone.After 4 weeks of treatment,Pittsburgh sleep quality index was used to evaluate the clinical therapeutic effect of the two groups,polysomnography was used to monitor the sleep quality of the two groups,and the differences of adverse drug reactions between the two groups were compared.Results:After 4 weeks of treatment,the total effective rate in the observation group was 95.65%,which was significantly higher than 67.39%in the control group(P<0.05).Before treatment,there was no significant difference in sleep quality between the two groups(P>0.05).The sleep time(381.56±75.23)min,sleep latency(19.24±11.47)min,rapid eye movement latency(151.36±58.24)min and sleep efficiency(85.54±6.65)%in the observation group were significantly better than those in the control group(P<0.05).There were 2 cases of nausea,2 cases of vomiting and 1 case of weight gain in the observation group.The total incidence was 10.87%,which was lower than 13.04%in the control group(P<0.05).Conclusion:Ezopiclone combined with olanzapine in the treatment of refractory insomnia can effectively improve the clinical efficacy and improve sleep quality.It is worthy of promotion.
